HELLP Sendromunda Görme Kaybı
Erdem Dinç, Umut Can Kurtuluş, Atila ArgınMersin Üniversitesi Tıp Fakültesi, Göz Hastalıkları Ana Bilim Dalı, MersinOn dokuz yaşında primigravid hastanın 37. gestasyon haftasında konvulsiyonları, frontal baş ağrısı ortaya çıktı ve hastada eklampsi düşünülerek sezaryen uygulandı. Postoperatif 10. günde hastanın her iki gözünde görme kaybı ortaya çıktı. Sağ gözde en iyi düzeltilmiş görme keskinliği 3/10, sol gözde ise 2 metreden parmak sayar düzeydeydi. Bilateral ön segment muayenesi doğal olan hastanın her iki fundus muayenesinde maküler hemoraji ve eksudalar izlendi. Yapılan laboratuar tetkiklerinde HELLP sendromu ile uyumlu olarak karaciğer enzimlerinde artış, trombositopeni tespit edildi. Takiplerde her iki maküladaki hemoraji geriledi ve hastanın görme düzeyi arttı. HELLP sendromunda çeşitli oftalmolojik komplikasyonların yanı sıra maküler hemoraji gelişimi izlenebilmektedir.

She was diagnosed eclampsy and cesarean section was implemented. The patient’s complaint was bilateral loss of vision on the postoperative 10th day. Best corrected vision acuity of the right eye was 3/10 while the left eye was 2 meters finger count. Bilateral anterior segment examination was normal. Examination of the both fundus displayed bilateral macular hemorrhage and exudates. In accordance with the HELLP syndrome, laboratory studies confirmed that the level of liver enzymes increased and thrombocytopenia was detected. HELLP syndrome may cause development of macular hemorrhage besides various ophthalmologic complications.
