Diyabetik Maküla Ödemli Tedavi Naif Gözlerde Ranibizumab veya Aflibercept Monoterapileri: Gerçek Yaşam Deneyiminde Birebir Karşılaştırma

Gereç ve Yöntemler: Bu retrospektif kohort çalışmasında, hastanemize başvuran DMÖ'lü tedavi naif olguların tıbbi çizelgeleri gözden geçirildi. DMÖ'lü toplam 512 tedavi naif göze ya ranibizumab (Grup I; 308 göz) ya da aflibercept (Grup II; 204 göz) monoterapisi uygulandı ve 462 hasta kaydedildi. 12 ay boyunca görsel kazanımları birincil sonuç olarak değerlendirildi.
Bulgular: Birinci yılın sonunda ortalama intravitreal enjeksiyon sayısı Grup I ve II'de sırasıyla 4.34±1.83 ve 4.39±2.12 idi (p=0.260). 12 ayda ortalama en iyi düzeltilmiş görme keskinliği (EİDGK) iyileşmesi Grup I ve II'de sırasıyla +5.7 ve +6.5 ETDRS harfleriydi (p=0,321); bununla birlikte, EİDGK skoru 69 ETDRS harfinin altında olan gözlerde (çalışma popülasyonunun %54'ü) Grup II'de görsel kazanım daha belirgindi (+12.1'e karşı +15.2 ETDRS harfleri; p<0.001). Ranibizumab veya aflibercept monoterapisi alan gözlerde istatistiksel olarak anlamlı santral foveal kalınlık (SFK) düşüşü gözlendi (p<0.001), her iki grup arasında anlamlı bir fark olduğuna dair herhangi bir kanıt yoktu (p=0.148).
Sonuç: PRN protokolüne göre uygulanan ranibizumab ve aflibercept monoterapileri arasında, aflibercept kolunda biraz daha iyi fonksiyonel ve anatomik prognoz eğilimi olmasına rağmen, 12 aylık takipte görsel sonuçlarda istatistiksel olarak anlamlı fark bulunmadı.
Ranibizumab or Aflibercept Monotherapies in Treatment-Naive Eyes with Diabetic Macular Edema: Head to Head Comparison in A Real-Life Experience

Materials and Methods: Medical charts of treatment-naive cases with center involved DME retrieved from our institutional database were reviewed in this retrospective cohort study. A total of 512 treatment-naïve eyes with DME underwent either ranibizumab (Group I; 308 eyes) or aflibercept (Group II; 204 eyes) monotherapy of 462 patients were enrolled. The primary outcome was visual gain over the 12-month.
Results: The mean number of intravitreal injections within the first year was 4.34±1.83 and 4.39±2.12 in Group I and II, respectively (p=0.260). The mean best corrected visual acuity (BCVA) improvement at 12 months was +5.7 and +6.5 ETDRS letters in Group I and II, respectively (p=0.321); however, visual gain was more prominent in Group II (+12.1 versus +15.2 ETDRS letters; p<0.001) in eyes with BCVA score less than 69 ETDRS letters (54% of the study population). Statistically significant central foveal thickness (CFT) decrease was observed in eyes received either ranibizumab or aflibercept monotherapy (p<0.001) without any evidence of a significant difference between both groups (p=0.148).
Conclusions: No statistically significant difference was found in visual outcomes at 12-month follow-up between ranibizumab and aflibercept monotherapies performed according to PRN protocol despite the tendency of slightly better functional and anatomic prognosis in aflibercept arm.
