Bipolar Bozukluk Tanılı Hastalar ve Sağlıklı Kardeşlerinin Şizotipi Özellikleri Açısından Karşılaştırılması

Boyutsal değerlendirmeyle bakıldığında şizotipi boyutunun normalden başlayıp sadece şizofreni spektrumu ile sonlanmadığı, bipolar bozukluk (BP) ve obsesif kompulsif bozukluk gibi diğer patolojilere de genişletilebileceği söylenebilir. Bu nedenle bu çalışmada, bir grup BP tanılı hastada şizotipal özelliklerin değerlendirilmesini ve BP tanılı hastalar ve kardeşlerinin şizotipal özellikler açısından karşılaştırılmasını amaçladık.
YÖNTEM ve GEREÇLER: Bipolar bozukluk tanı ölçütlerini karşılayan, remisyonda 31 birey ve herhangi bir psikiyatrik tanı konmamış olan 31 sağlıklı kardeş çalışmaya alındı. BP hastaları ve sağlıklı kardeşlerine Şizotipal Kişilik Ölçeği (ŞTKÖ) uygulandı.
BULGULAR: ŞTKÖ toplam skorları açısından karşılaştırmada hastaların skorlarının sağlıklı kardeşlerden anlamlı düzeyde yüksek olduğu belirlenmiştir (p<0,001). Şizotipi ölçek skorlarının hasta ve kardeşler arasında karşılaştırmasında dokuz alt ölçekten altısında hasta grubu skorlarının sağlıklı kardeşlerine göre anlamlı düzeyde yüksek olduğu gözlenmiştir. Üç boyutlu değerlendirmede bilişsel algısal şizotipi ve kişilerarası şizotipi skorları hastalarda anlamlı düzeyde yüksek görülürken (p<0,001), iki boyutlu değerlendirmede ise hem pozitif hem de negatif skorlarda hastaların ölçek skorlarında anlamlı yükseklik olduğu belirlendi (sırayla; p<0,001; p<0,001). Hasta ve kardeşlerinin ölçek puanları arasında da korelasyon olduğu gözlendi.
TARTIŞMA ve SONUÇ: Çalışmamızda BP tanılı hastaların ŞTKÖ toplam ve alt skorlarının kardeşlere göre yüksek olduğu ve bu sonucun kardeşleriyle korele şekilde ortaya çıktığı gösterilmiştir.
Comparison of Schizotypal Characteristics of Bipolar Patients and Their Healthy Siblings

When we look at the dimensional evaluation, it may be suggested that the size of schizotypic dimension can be extended to other pathologies such as bipolar and obsessive compulsive disorder which do not end with the schizophrenia spectrum. Therefore, in this study, we aimed to evaluate the schizotypal features in a group of BP patients and to compare BP patients and their siblings in terms of schizotypal features.
METHODS: Thirty-one bipolar patient and 31 healthy siblings who did not have any psychiatric diagnosis were included in the study. BP patients and healthy siblings were administered the Schizotypal Personality Questionnaire.
RESULTS: Total schizotype scores of the patients were significantly higher than healthy siblings (p<0.001). In the comparison of scale scores between patients and siblings, it was observed that six of nine subscales of patient group were significantly higher than the scores of healthy siblings. According to three dimenisonal evaluation, cognitive perceptual and interpersonal schizotypy scores were found to be significantly higher in the patients (p<0.001). Besides, negative and positive schizotype scores were also higher in patient group (p<0.001). It was observed that the scale scores of the patients and their siblings were correlated.
DISCUSSION AND CONCLUSION: Our study showed that the total and subscores of Schizotypal Personality Questionnaire for BP patients were higher than their siblings, and that this result was correlated with the siblings.
