Fasiyal paralizi görülen bir Malign Otitis Eksterna olgusu
Belgin Tutar, Güler Berkiten, Ayça Tazegül Mutlu, Ayşe Enise Göker, Yavuz UyarOkmeydanı Eğitim Ve Araştırma Hastanesi Kbb, İstanbulDış kulak kanalından başlayarak temporal kemik, mastoid hücreler ve kulak çevresindeki yumuşak dokulara yayılabilen Malign Otitis Eksterna nadir görülmesine ragmen, şiddetli seyreden destrüktif bir hastalıktır. İmmünsupresyon ve Diabetes Mellitus önemli predispozan faktörlerdir. En sık etkenin Pseudomonas türleri olduğu enfeksiyon, akut otitis eksternadan ayırt edilmelidir. Erken tanı ve tıbbi tedavi ile başarı oranları yüksektir. Çalışmamızda sol kulak ağrısı, akıntısı ve sol yüz felci yakınmaları nedeniyle ünitemize başvuran; 55 yaşında diabetik erkek hasta sunuldu. Yapılan fizik muayene, Teknesyum 99m kemik sintigrafisi ve temporal kemik Bilgisayarlı Tomografisi ile malign otitis eksterna tanısı kondu. Medikal tedavi ile iyileşme sağlandı. Bulgular literatür eşliğinde tartışıldı.

Immunosuppression and Diabetes Mellitus are the important predisposing factors. The infection whose the most frequent factor is Pseudomonas types, should be distinguished from acute otitis externa. Success rates are high in early diagnosis and medical treatment. A 55-year-old diabetic male patient, who applied to our unit with the complaints of left ear ache, discharge and left side of facial paralysis, has been diagnosed with malignant otitis externa, by physical examination, technetium-99m bone scintigraphy and computed tomography of the brain in axial plane, and recovered by medical treatment. Diagnosis was discussed by the literature.
