Androgenetik Alopesi Tedavisinde Plateletten Zengin Plazma Enjeksiyonu Klinik Sonuçlarımız

GİRİŞ ve AMAÇ: Plateletten zengin plazma (PZP) normal kandaki platelet kosantrasyonunun optimum değerlere çıkarıldığı küçük bir plazma hacmini temsil eder. Etkisi plateletlerden salınan büyüme faktörlerine bağlıdır. Plastik cerrahinin neredeyse birçok alanında araştırılmış ve kullanılmış olmakla birlikte son dönemde saç dökülmesi tedavisinde ilgi odağı haline gelmiştir. Bu çalışmada PZP hazırlanışı ve uygulanması için standart bir protokol oluşturularak erkek ve kadın alopesisi olan hastalarda adjuvan tedavideki etkinliğini değerlendirmek amaçlanmıştır.
YÖNTEM ve GEREÇLER: Hafif-orta androgenetik alopesi (AGA) (kadınlar için Ludwig alopesi skoru I-II ve erkekler için Hamilton-Norwood skoru 1-4) mevcut olan ve son 6 ay içinde topikal veya sistemik alopesi tedavisi almamış olan 15 hasta çalışmaya dahil edildi. Hastaların 11’i erkek ve 4’ü kadındı, ortalama yaş 41’di (27-55). PZP tek çevirmeli metodla yüksek ivmelenme kuvveti ve düşük santrifüj süresi ile hazırlanmış olup 3-4 haftalık aralarla toplam 3 doz PZP enjeksiyonu yapıldı. Hasta memnuniyeti Vizüel Analog Skala (VAS) ile değerlendirildi. 0. gün, 1.ay, 2.ay ve 6.aydaki değerlendirmeler retrospektif olarak incelendi.
BULGULAR: Tedavi öncesi tüm hastalarda saç çekme testi pozitifti (%100) ve ele gelen saç kaybı sayısı ortalama 6,7 idi. Üç uygulama sonrası saç çekme testi 13 hastada negatifti (%86,6) ve ele gelen ortalama saç sayısı 2.5 olarak bulundu. İşlem öncesi ve sonrası standart bir şekilde çekilen makroskobik fotoğraflar ile saç yoğunluğu ve kalitesinde iyileşme saptandı. Hasta memnuniyet değerlendirmesinin yapıldığı VAS da işlem öncesi değer 6.8 iken, işlem sonrası 2.1’e gerilemiştir.
TARTIŞMA ve SONUÇ: Ticari kit kullanmaksızın basit bir şekilde hazırlanıp periyodik olarak uygulanan PZP enjeksiyonları, yüksek hasta memnuniyeti ve düşük maliyeti ile alopesi tedavisinde etkili bir tedavi seçeneğidir.
Clinical Outcomes of Platelet-rich Plasma Injections in the Treatment of Androgenetic Alopecia

INTRODUCTION: Platelet-rich Plasma (PRP) is an autologous suspension of platelets in a small volume of plasma characterized by a platelet concentration above basal original blood values collected. It’s effect depends on the growth factors released from the platelets. Although it has been investigated and used in most areas of plastic surgery, PRP injection has recently attracted interest in the treatment of hair loss. In this study, we aimed to evaluate the efficacy of adjuvant therapy in patients with male and female alopecia by establishing a standard protocol for the preparation and application of PRP.
METHODS: Fifteen patients with mild to moderate androgenetic alopecia (AGA) (Ludwig alopecia score I-II for women and Hamilton-Norwood score 1-4 for men) who had not received topical or systemic treatment for alopecia in the last 6 months were included in the study. Eleven of the patients were male and 4 were female, with a mean age of 41 (27-55). PRP was prepared using a single spin method with high acceleration force and low centrifugaiton time, and three sessions were performed with an interval of 4 weeks. Patient satisfaction was assessed by visual analogue scale (VAS). All patients were evaluated at beginning of the study, 1 month, 2 months and 6 months, respectively.
RESULTS: Hair pull test was positive (100%) in all patients before the treatment, and average number of hair loss was 6.7. After the third session, the pull test was negative in 13 patients (86,6%) with average number of 2.5 hairs. An improvement in hair density and quality was showed with macroscopic photographs taken before improvement in hair density and quality. Average VAS reduced from 6.8 to 2.1 after the procedure.
DISCUSSION AND CONCLUSION: Prp injections together with a simple preparation and periodical application without using commercial kit have a positive effect for the treatment of alopecia with high patient satisfaction and low cost.
