Covid-19 Hastalarında İmmün Plazma Tedavisinin Prognoz ve Mortalite Üzerine Etkisi

GİRİŞ ve AMAÇ: Covid-19 pandemisinde tedavi tam olarak bilinmediğinden geleneksel yöntemlerden biri olan İmmün Plazma da yaygın uygulandı. Bu çalışma ile amacımız; yoğun bakım ünitemize Covid-19 tanısı ile yatan ve immün plazma tedavisi uygulanan hastalarda tedavinin prognoz ve mortalite üzerine etkisini incelemektir.
YÖNTEM ve GEREÇLER: Hastanemiz Anesteziyoloji ve Reanimasyon Kliniğine bağlı yoğun bakım ünitelerinde, Covid-19 tanısı PCR testi ile doğrulanmış, 60 yaş üstü veya 18-60 yaş arası komorbid hastalığı bulunan ve immün plazma tedavisi alan 209 hastanın dosya ve elektronik ortam kayıtlarını retrospektif olarak inceledik. Kontrol grubu olarak İmmün plazma tedavisi verilmeyen benzer demografik verilere sahip 50 hasta dosyası inceledik. Hastaların demografik verileri, yoğun bakım ünitesine kabul edildiği gün, yatış gün sayısı, eşlik eden hastalıkları, semptom süresi ve immün plazma tedavi öncesi ve sonrası lökosit sayısı, lenfosit sayısı, nötrofil sayısı, C-reaktif protein, prokalsitonin, kreatin, D-Dimer, fibrinojen, ferritin, IL-6,PaO2/FiO2, oksijen gereksinimi ve ventilasyon durumu, mortalite ve morbidite değerlendirilerek kaydettik. Bu verileri Covid-19 tanılı immün plazma tedavisi alan hastalarda mortalite ve prognoz öngörüsü açısından istatistiksel olarak analiz ettik.
BULGULAR: Laboratuar bulgularında anlamlı düzelme olsa da, çalışma grubumuza dahil edilen 209 hastadan %62.2 (n=132)’si, kontrol grubu hastalarından %62 (n=31)’i eksitus olmuştur
TARTIŞMA ve SONUÇ: Şiddetli Covid-19 tanılı hastalarda immün plazma tedavisi kurtarma tedavisi olarak fayda sağlamaz ve mortaliteyi azaltmaz.
The Effect of Immune Plasma Therapy on Prognosis and Mortality in Covid-19 Patients

INTRODUCTION: Since the exact treatment was not known in the Covid-19 pandemic, one of the traditional methods, Immune Plasma therapy, was also widely used. Our aim with this study is to examine the effect on prognosis and mortality of the patients hospitalized in the intensive are unit with the diagnosis of Covid-19 and receiving immune plasma therapy.
METHODS: We retrospectively analyzed the files and electronic media records of 209 patients over the age of 60 or between the ages of 18-60, with comorbid diseases and who received immune plasma therapy in the intensive care units of our hospital's Anesthesiology and Reanimation Clinic, whose diagnosis of Covid-19 was confirmed by PCR test. As a control group, we analyzed the files of 50 patients with similar demographic data who were not given immune plasma therapy. We recorded demographic data of the patients, the day of admission to the intensive care unit, the number of days of hospitalization, comorbid diseases, duration of symptoms and lab parameters of leukocyte count, lymphocyte count, neutrophil count, C-reactive protein, procalcitonin, creatine, D-Dimer, fibrinogen, ferritin, IL-6; also PaO 2 /FiO 2 ratio, oxygen requirement and ventilation status before and after immune plasma treatment. We statistically analyzed these data in terms of mortality and prognosis prediction in patients diagnosed with Covid-19 who received immune plasma therapy.
RESULTS: Despite significant improvement in laboratory findings, 62.2% (n=132) of the 209 patients in our study group and 62% (n=31) of the control group died.
DISCUSSION AND CONCLUSION: Immune plasma therapy does not provide benefit as a rescue treatment and does not reduce mortality in patients with severe Covid-19.
