Surgical Treatment of Obesity

As a cause of many co-morbidities, psychosocial problems in many patients and huge financial load for the treatment are main concerns in this disease. Medical management is usually not enough to loose weight. Surgery has become main part of the management of morbid obesity. During the last 60 years some surgical techniques are being used for this purpose. Roux-en-Y gastric bypass, sleeve gastrectomy, adjustable gastric banding and biliopancreatic diversion are the most commonly performed up-to-date surgical procecures. All of these could be performed laparoscopically. With the surgery; less food intake and early satiety due to gastric volume restriction and malabsorbtion due to bypass cause excess weight loss.

Birçok morbiditeye neden olduğu gibi hastalarda oluşturduğu psikososyal sorunlar ve tedavi için büyük mali yük getirmesi bu hastalıkta ana sorunlardır. Kilo kaybı için medikal tedavi genellikle yetersizdir. Cerrahi, morbit obezite tedavisinin ana parçası haline gelmiştir. Bu amaçla geride kalan 60 yıllık sürede bazı cerrahi teknikler kullanılmaktadır. Roux-en-Y gastrik bypass, sleeve gastrektomi, ayarlanabilir gastrik band ve biliopankreatik diversiyon en sık uygulanan güncel cerrahi işlemlerdir. Bu işlemlerin tümü laparoskopik olarak gerçekleştirilebilir. Cerrahi ile mide hacminin küçülmesine bağlı az yeme ve erken doyma, bypass sonucu emilimin bozulması fazla kilo kaybına neden olur.
