The role of fine needle aspiration biopsy in the diagnosis of parotis masses

OBJECTIVE: To determine the utility of fine needle aspiration biopsy (FNA) in the evaluation of parotit lesions by comparing the results of preoperative FNA and postoperative pathology report results
METHODS: Data from 128 patients who were operated for parotid masses and on who underwent preoperative FNA was analyzed retrospectively between 2009-2014. Specificity and sensitivity values of FNA were calculated.
RESULTS: 84 of the patients were male (%65,6) and 44 were female (%34,3). Mean age was 48 (range: 13-76). FNA was insufficient in 16 of the 128 patients (%12). 96 of the other 112 patients were reported as benign. 88 of these (%91) were confirmed by postoperative histopathology. Of the 16 patients who were reported as malignant by FNA, 12 of them were confirmed postoperatively (%75). In evaluating histopathologically confirmed lesions, the absolute sensitivity in our patients was %75 and the specificity was %95,4. The positive predictive and the negative predictive values were %75 and %87,5 respectively. The accuracy rate was %85,7.
CONCLUSION: Fine needle aspiration biopsy(FNA) is a reliable diagnosing method and it is important to determine of the treatment of parotid masses in the preoperative period.

AMAÇ: Parotis ince iğne aspirasyon biyopsisi (İİAB) sonuçları ile opere edilen vakaların histopatolojik sonuçları karşılaştırılarak, parotis lezyonlarının değerlendirilmesinde İİAB’ nin tanısal değerinin incelenmesidir.
YÖNTEMLER: 2009-2014 yılları arasında parotis kitlesi nedeniyle opere edilen 128 hastanın preoperatif İİAB ile postoperatif histopatolojik sonuçları karşılaştırıldı. İİAB’nin tanı koydurucu bir test olarak duyarlılık ve özgüllük oranları incelendi.
BULGULAR: Hastaların 84’ü erkek (%65,6), 44’ü kadın (%34,3) olup yaş ortalaması 48 ( 13-76 yaş arası) idi. İİAB, 128 hastanın16 sında yetersiz olarak değerlendirildi (%12). 112 hastanın 96’sında İİAB benign olarak değerlendirildi. Bunların 88’nin postoperatif histopatolojisinde benign olarak tanı kondu. (%91). Histopatolojik olarak doğrulanan lezyonların değerlendirilmesinde, hastalarımızdaki duyarlılık %75, özgüllük ise %95.4 olarak bulundu. Pozitif prediktif değer (PPD) ve negatif prediktif değer (NPD)’ ler sırasıyla %75 ve %87.5 olarak saptandı; doğruluk ise %85,7 idi.
SONUÇ: İnce iğne aspirasyon biyopsisinin etkili ve güvenli bir teşhis yöntem olarak parotis bezi kitlelerinin preoperatif incelemesinde yapılması önerilir.
