INTRODUCTION: Purpose: The aim of this study is to compare the short-term efficacy of dexamethasone (Dex) implant with those of three consecutive ranibizumab (Rzb) injections in treatment-naive diabetic macular edema patients METHODS: Methods: In this retrospective study, 30 eyes of 30 diabetic macular edema (DME) patients were enrolled into the ranibizumab (IVR) group; 29 eyes of 29 patients were enrolled into the Dex (DI) group. The R group was treated with 3 consecutive monthly ranibizumab injections; the DI group was injected one single Dex implant. Both groups underwent ophthalmological examinations and optical coherence tomography (OCT) exams at baseline and 1st, 2nd and 3rd month visits. Data were analyzed with SPSS 22.0 statistically. RESULTS: Results: Mean age, duration of diabetes, baseline best-corrected visual acuity (BCVA) and central macular thickness (CMT) of both arms were statistically indifferent. The intraocular pressure (IOP) changes were within and between groups insignificant. The comparison of visual gain at final visit showed no difference (p>0,05). The final CMT reduction in both groups was statistically insignificant (145µm vs 110.5µm p>0,05). In DI group; 2 eyes (7%) had to be treated with topical anti-glaucomatous agents in the follow-up. DISCUSSION AND CONCLUSION: Conclusion: Both monthly ranibizumab injections and dexamethasone implant are equally effective in the initial treatment of diabetic macular edema. Although the treatment effect of Dex implant was seen earlier in the follow-up, the IOP elevations may be a clinical concern in some cases.

GİRİŞ ve AMAÇ: Bu çalışmanın amacı tedavi naif diyabetik makular ödem hastalarında deksametazon implant (DI) ile üç ardışık ranibizumab enjeksiyonlarının kısa dönem etkinliğinin karşılaştırılmasıdır. YÖNTEM ve GEREÇLER: Bu retrospektif çalışmada, 30 diyabetik makular ödem (DMÖ) hastasının 30 gözü intravitreal ranibizumab (IVR) grubuna; 29 hastanın 29 gözü DI grubuna dahil edilmiştir. IVR grubu üç ardışık aylık ranibizumab enjeksiyonu ile tedavi edilirken; DI grubuna bir doz deksametazon implant enjekte edilmiştir. Her iki grup başlangıç, 1.ay, 2.ay ve 3.ayda oftalmolojik muayeneden ve optik koherens tomografi (OKT) incelemelerinden geçmiştir. Veriler istatistiksel olarak SPSS 22.0 ile analiz edilmiştir.
BULGULAR: Her iki çalışma kolu ortalama yaş, diyabetin süresi, en iyi düzeltilmiş görme keskinliği (EDGK) ve merkezi makular kalınlık (MMK) açısından benzerdi. Gruplar arası ve grup içi göz içi basıncı (GIB) değişimleri istatistiksel olarak benzerdi. Son muayenedeki görme kazanımları karşılaştırıldığında fark bulunamadı (p>0,05). Son muayenede her iki gruptaki MMK azalması benzer bulundu (145µm vs 110.5µm p>0,05). Takiplerde DI grubundaki iki gözde (%7) topikal antiglokomatöz ajan tedavisi gerekti. TARTIŞMA ve SONUÇ: DMÖ'in başlangıç tedavisinde aylık IVR enjeksiyonları ve DI benzer etkinliğe sahiptir. DI tedavi edici etkisi daha erken ortaya çıkmaktadır ancak GIB yükselmesi bazı olgularda dikkat edilmesi gereken bir durumdur.
