Transuretral Hiperterminin Prostat Hiperplazileri Tedavisindeki Yeri

Prostat 180 dakikalık tek bir seansta 44,5 °C'a kadar transuretral olarak ısıtıldı. Yapılan 1-7 aylık takipte; semptomatik benign prostat hiperplazili hasta grubunda ortalama idrar akım hızında ve maksimum idrar akını hızında belirgin artma (p < 0,0001) saptandı. Çok iyi sonuç (% 50,5), iyi sonuç (% 42,1), prostatektomi (%7,3) sonuçları gözlemlendi. İdrar retansionu olan sondalı grupta ise sondadan kurtulma (% 61,9), sondalı olarak devam eden (% 20,9), prostatektomi (% 17,1) oranda saptandı. Hematüri (% 30), mesane spazmı (% 26), ısıya duyarlılık (% 19), tedavi sırasında analjezi gerektiren perineal ağrı (% 15) oranda görüldü. Kişisel veya tıbbi nedenlerden dolayı cerrahi tedavi uygulanamayan benign prostat hiperplazili hastalarda transuretral hipertermi ayaktan yapılabilen güvenli ve umut vaad eden bir tedavi şekli olarak görülmektedir.

The prostate was heated transurethrally to 44,5 ° C with the treatment consisting of 1 session of 180 minutes. Follow up ranged from 1 to 7 months. Significant improvement include increased mean urine flow rate and maximum urine flow rate (p<0,0001). We observed for the symptomatic benign prostatic hypeı-plasia group; better response (% 50,5), good response (% 42,1), prostatic resection (% 7,3). We noted for the urinary retention group with the indweiling catheters; free from the catheter (% 61,9), continue with catheter (% 20,9), prostatic resection (% 17,1). Toxicity was mild and consisted of hematuria (% 30), bladder spasms (% 26), heating sensation (% 19), perineal pain requiring analgesics during treatment (% 15). Transurethral hyperthermia represents a safe and promising outpatient approach to the treatment of benign prostatic hyperplasia, particularly for patients who are not candidate for conventional surgical approaches because of medical or personal reasons.
