Progression of Disc Herniation After Transforaminal Epidural Steroid Injection: Should It be Progression or Regression?

Transforaminal epidural steroid injection (TFESI) treatment usually provides improvement in pain and functionality, also some studies have shown that epidural steroid injection can provide resorption of disc material. We aimed to report a case of a progression of disc herniation after TFESI and inform our colleagues about this possible situation. A 35-year-old female patient was admitted to us with complaints of low back pain radiating to the left leg. TFESI was applied to the patient's left L4-5 and L5-S1 disc levels. On the 2nd day after the procedure, the patient's pain increased, weakness in the left leg was observed, and the progression of the disc herniation at the L4-5 level was detected. However, prospective controlled studies are needed to determine whether this is a possible side effect of treatment or not.

Transforaminal epidural steroid enjeksiyonu (TFESE) tedavisi genellikle ağrı ve işlevsellikte iyileşme sağlamakla birlikte disk materyalinin rezorpsiyonunu da sağlayabildiğini gösteren çalışmalar da mevcuttur. TFESE sonrası disk herniasyonunda progresyon gelişen bir olguyu sunmayı ve bu olası durum hakkında meslektaşlarımızı bilgilendirmeyi amaçladık. 35 yaşında kadın hasta bel ve sol bacağa yayılan ağrı şikayeti ile tarafımıza başvurdu. Hastanın sol L4-5 ve L5-S1 disk seviyelerine TFESE uygulandı. İşlem sonrası 2. günde hastanın ağrılarında artış, sol bacakta güçsüzlük gözlendi ve çekilen MR'da L4-5 seviyesinde disk herniasyonunda progresyon olduğu saptandı. Ancak bu durumun TFESE tedavisinin olası bir komplikasyonu olup olmadığını belirlemek için prospektif kontrollü çalışmalara ihtiyaç vardır.
