1. | Yoğun Bakım Ünitesinde Yatan Multi Travmalı Hastaların Retrospektif İncelenmesi Retrospective Investigation of Multitrauma Patients in ICU Orhun Demir, Tuba Berra SarıtaşSayfalar 207 - 213 GİRİŞ ve AMAÇ: Skorlama sistemleri; yaralanma şiddetini belirlemek, triaj yapabilmek, epidemiyolojik verilerin oluşturulmasında kullanılabilirler. Çalışmamızın amacı; skorlama sistemlerinin performansını değerlendirmektir. YÖNTEM ve GEREÇLER: Multitravma tanısıyla yoğun bakım ünitesinde tedavi edilen 130 hasta çalışmaya alındı. Hastaların demografik özellikleri, mekanik ventilasyonda ve yoğun bakımda kalış süreleri, mortalite oranları ve skorlama sistemlerinden aldığı puanlar hesaplandı ve kaydedildi. BULGULAR: Hastaların yaş grupları ile mortalite arasındaki ilişki incelendiğinde 65 yaş üzeri hastalarda mortalite yüzdesinin (%65,2), 18-65 yaş arası hastalardaki (%36,7) ve 18 yaş altı hastalardaki mortalite yüzdesinden (%23,5) istatistiksel olarak anlamlı bir şekilde daha yüksek olduğu bulunmuştur (p<,05). Yaşayan hastaların ortalama; GKS, RTS, APACHE II, TRISS, ISS, SAPS 2, Genişletilmiş SAPS 2 ve SOFA skorları sırasıyla; 9.94±4.09, 5.95±1.34, 11.46±6.18, 20.30±7.45, 14.86±10.20, 24.27±15.33, 3.98±1.34, 4.81±1.52’dir. Mortalite gelişen hastaların ortalama skorları aynı sıra ile; 5.40±2.19, 2.95±0.70, 31.96±6.79, 40.19±9.51, 43.60±16.95, 62.73±13.96, 5.69±1.88, 6.54±4.20’dir. Hastaların yoğun bakımda kalış süreleri ortalama 11,55±16,12 gün olarak saptanmıştır. TARTIŞMA ve SONUÇ: Yoğun bakımda kullanılan skorlama sistemleri karşılaştırıldığında; GKS, RTS, TRISS, APACHE II, SAPS 2 ve geniş SAPS 2 mortalite tahmininde bir belirteç olarak kullanılabilir. SOFA skoru ile mortalite tahmini arasında anlamlı bir sonuç elde edilememiştir. Çalışmamızda ki tüm skorlama sistemleri incelendiğinde yoğun bakımda ve mekanik ventilasyonda kalış sürelerinin tahmininde verimli skorlama sistemleri olduğu görülmüştür INTRODUCTION: Scoring systems; they can be used to determine the severity of injury, to triage, to create epidemiological data. The average length of stay in the intensive care unit was found to be 11.55 ± 16.12 days DISCUSSION AND CONCLUSION: When the scoring systems used in intensive care are compared; GKS, RTS, TRISS, APACHE II, SAPS 2 and broad SAPS 2 can be used as a predictor of mortality. A meaningful result could not be obtained between the SOFA score and mortality prediction. When all scoring systems in our study were examined, it was seen that there were efficient scoring systems in predicting the duration of stay in intensive care and mechanical ventilation |
