Ortotopik karaciğer transplantasyonu hastalarında perioperatif yönetimi
Melis Dulgeroglu, Nedim ÇekmenGüven Hastanesi, AnkaraGünümüzde ortotopik karaciğer transplantasyonu (OKT) karaciğer yetmezliği hastalarında en kesin tedavi şeklidir. OKT sonrası postoperatif erken dönemde hemodinaminin stabilleşmesi, mekanik ventilasyon desteği ve doğru weaning ile oksijenizasyonun sağlanması, renal fonksiyonların korunması, hemostazın sağlanması temel hedeflerimiz olmalıdır. Tecrübeli bir yoğun bakım ekibi ile gelişebilecek komplikasyonların erken tanı ve tedavisi greft fonksiyonları ve sağ kalım açısından önemlidir. Bu derlemede güncel literatür doğrultusunda OKT hastalarında perioperatif yoğun bakım yönetiminden bahsedildi.

Early hymodynamic stabilization, maintenance of oxygenation with mechanical ventilation and weaning, protect renal functions, providing hemostasis should be our main targets. Early diagnosis and treatment of complications with experienced intensive care team is important for graft function and survival. In this review, the perioperative intensive care management of OLT patients was subjected according to actual literature.
