The effects of corticosteroid use on prognosis of patients with nosocomial meningitis in neurosurgical intensive care unit: A retrospective descriptive study

INTRODUCTION: Neurosurgical intensive care unit patients are at a higher risk of nosocomial meningitis due to invasive procedures, such craniotomies and external ventricular drainage catheters, and penetrating head traumas. While some studies suggest that corticosteroids increase this risk, others argue no such effect exist. This retrospective descriptive study aims to evaluate how corticosteroid use influences mortality, nosocomial infections, number of surgical interventions and length of stay in intensive care unit of neurosurgical intensive care unit patients with nosocomial meningitis.
METHODS: Thirteen nosocomial meningitis cases were identified among 2228 patients admitted to neurosurgical intensive care unit over the course of 5 years and were retrospectively analyzed for corticosteroid use, mortality, nosocomial infections, number of surgical interventions and length of stay in intensive care unit.
RESULTS: Corticosteroid use does not increase mortality, nosocomial infections, number of surgical interventions and length of stay in intensive care unit in patients with nosocomial meningitis.
DISCUSSION AND CONCLUSION: Corticosteroid use of neurosurgical intensive care unit patients with nosocomial meningitis does not affect mortality, nosocomial infections, number of surgical interventions and length of stay in intensive care unit.

Nozokomiyal meninjit gelişen beyin cerrahisi yoğun bakım hastalarında kortikosteroid kullanımının prognoz üzerine etkisi: Retrospektif tanımlayıcı çalışma

GİRİŞ ve AMAÇ: Beyin cerrahisi yoğun bakım hastalarında kraniyotomiler, eksternal ventrikül drenaj kateteri gibi invaziv uygulamalar ve penetran kafa travmaları nozokomiyal meninjit gelişme riskini arttırmaktadır. Kortikosteroid kullanımının da bu riski arttırdığını gösteren çalışmalar olduğu gibi tersini savunan çalışmalar da bulunmaktadır. Bu retrospektif tanımlayıcı çalışmanın amacı 5 yıl süresince beyin cerrahisi yoğun bakım ünitesinde nozokomiyal meninjit gelişen hastalarda kortikosteroid kullanımının mortalite, nozokomiyal enfeksiyon, cerrahi girişim sayısı ve yoğun bakım yatış süresi üzerine etkisini ortaya koymaktır.
YÖNTEM ve GEREÇLER: Bu çalışmada 5 yıllık sürede beyin cerrahisi yoğun bakım ünitesinde yatan 2228 hastadan nozokomiyal meninjit gelişen 13 olgu retrospektif olarak incelendi. Nozokomiyal meninjit gelişen hastalarda kortikosteroid kullanımının mortalite, nozokomiyal enfeksiyon, cerrahi girişim sayısı ve yoğun bakım yatış süresi üzerine etkisi araştırıldı.
BULGULAR: Kortikosteroid kullanımının nozokomiyal meninjiti olan hastalarda mortalite, nozokomiyal enfeksiyon riski, cerrahi girişim sayısı ve yoğun bakım yatış süresi üzerine etkisi bulunmadı (p>0.05).
TARTIŞMA ve SONUÇ: Nozokomiyal meninjiti olan beyin cerrahisi yoğun bakım hastalarında, kortikosteroid kullanımı; mortalite, nozokomiyal enfeksiyon riski, cerrahi girişim sayısı ve yoğun bakım yatış süresini etkilememektedir.
