Intoxications in Intensive Care; Cost and Bed Occupancy According to Glasgow Coma Scale

INTRODUCTION: Intensive care units (ICU) are the units in which critical care and follow-up are conducted. About 3,7-40% of ICU beds are used for intoxication. In ICU, intoxications may be mortal or can be discharged only by observation without any complication. The necessity of hospitalization of all of these patients to ICU should be discussed in terms of bed occupation and cost. In our study, it was aimed to determine the necessity and cost of hospitalization of ICU patients who were admitted to an intensive care unit from the emergency service with intoxication diagnosis.
METHODS: This study was conducted by reviewing the files of 205 intoxication patients who underwent more than 24 hours of follow-up and treatment at 2nd and 3rd level ICU. Patients were divided into two groups according to Glasgow Coma Score (GCS) 15 (Group= 15) and below 15 (Group <15) admitted to ICU. The patients' age, gender, GCS, poisoning cause, number of days spent in the ICU, and the need for mechanical ventilation (MV) were recorded and the ICU costs of the groups were calculated. In addition, the cost that would be generated if the patients in Group = 15 were followed in the service environment rather than in the ICU was calculated.
RESULTS: Of the 205 patients examined, 145 were GCS: 15, while 60 were below GCS 15. The number of patients with GCS = 15 and the incidence of suicide intoxication were higher in females. The number of intoxications with cardiovascular system drugs and analgesic drugs was greater in group = 15, while the number of intoxications with carbon monoxide and drug substances was greater in group <15. The number of days spent in ICU, MV requirement and ICU costs were higher in Group <15. If the patients in group = 15 had been followed in the service environment, the cost would have been lower than the cost in ICU.
DISCUSSION AND CONCLUSION: As GCS is specific and easy to administer, it can be used to determine the need of intoxication cases for admission to ICU units. It is widely believed that intoxications that are life threatening with organ insufficiency should be followed in 2nd and 3rd level ICUs. The fact that ICU beds in our country are not used according to the criteria is a big problem, which may increase the cost of use and cause an increase in mortality. Adhering to the criteria for accepting patients to high cost units which require specialist and technological equipment such as ICUs will prevent unnecessary bed occupation and ensure proper use of resources. According to our study, close follow-up of patients with GCS = 15 admission to ICUs at an equipped service would reduce cost and bed occupancy.

Yoğun Bakımda İntoksikasyonlar; Glasgow Koma Skoruna Göre Maliyet ve Yatak İşgali

GİRİŞ ve AMAÇ: Yoğun bakım üniteleri (YBÜ) kritik hasta bakımı ve takibinin yapıldığı birimlerdir. YBÜ yataklarının %3,7-40 kadarı intoksikasyonlar için kullanılmaktadır. YBÜ’de intoksikasyonlar mortal seyredebileceği gibi hiçbir komplikasyon olmaksızın sadece gözlemle taburcu edilebilmektedir. Bu hastaların tümünün YBÜ’ne yatışının gerekliliği, yatak işgali ve maliyet açısından tartışmalıdır. Çalışmamızda YBÜ’de intoksikasyon tanısıyla yatırılan hastaların YBÜ’ne yatış gerekliliği ve maliyetinin belirlenmesi amaçlanmıştır.
YÖNTEM ve GEREÇLER: Bu çalışma 2. ve 3. düzey YBÜ’de 24 saatten fazla takip ve tedavisi yapılan 205 intoksikasyon hastasının dosyalarının incelenmesiyle yapıldı. Hastalar YBÜ’ne kabul edilişindeki Glasgow Koma Skoru (GKS) 15 (Grup=15) ve 15’in altında (Grup<15) olmak üzere göre iki gruba ayrıldı. Hastaların yaş, cinsiyet, GKS, zehirlenme nedeni, YBÜ’nde yattığı gün sayısı, mekanik ventilasyon (MV) ihtiyacı kaydedildi ve grupların YBÜ maliyetleri hesaplandı. Ayrıca Grup=15’teki hastaların YBÜ’nde değil de servis ortamında takip edilmesi halindeki oluşturacağı maliyet hesaplandı.
BULGULAR: İncelenen 205 hastanın 145’inin GKS: 15 iken, 60 hastanın GKS 15’in altındaydı. GKS=15 olan hasta sayısı ve suisid amaçlı intoksikasyon olgusu kadın cinsiyette daha fazlaydı. Kardiyovasküler sisteme etkili ilaç ve analjezik ilaçla intoksikasyon sayısı Grup=15’te daha fazla iken karbonmonoksit ve uyuşturucu maddelerle oluşan intoksikasyon sayısı Grup<15’te daha fazlaydı. YBÜ’nde yattığı gün sayısı, MV ihtiyacı ve YBÜ maliyetleri Grup<15’te daha fazlaydı. Grup=15’teki hastalar servis ortamında takip edilseydi oluşacak maliyet YBÜ’de oluşan maliyetten daha düşük bulundu.
TARTIŞMA ve SONUÇ: GKS; spesifik olması ve kolay uygulanabilirliğiyle intoksikasyon olgularının YBÜ’ne yatışının gerekliliğinin belirlenmesinde kullanılabilir. Hayati tehlike ve organ yetmezliği yapan intoksikasyonların 2. ve 3. düzey YBÜ’lerinde takip edilmesi gerektiği yaygın kanıdır. Ülkemizde YBÜ yataklarının kriterlere uygun kullanılmaması büyük sorundur. Kriterlere uygun olmayan kullanım maliyeti artırabileceği gibi mortalite artışına da neden olabilir. YBÜ’leri gibi uzmanlık ve teknolojik donanım gerektiren, yüksek maliyetli ünitelere hasta kabulünde kriterlere uyulması, gereksiz yatak işgalini önleyecek ve kaynakların doğru kullanımını sağlayacaktır. Çalışmamıza göre YBÜ’ne kabulde GKS=15 olan hastaların donanımlı bir serviste yakından takip edilmesi maliyet ve yatak işgalini azaltabilecektir.
