The impact of LEEP and cold-knife conisation training model on the surgical skills and confidence level

Background: To evaluate the impact of loop electrosurgical excision procedure (LEEP) and cold-knife conisation (CKC) training model on the surgical education and confidence level of gynecologists.

Methods: LEEP and CKC hands on training model was consisted of a sausage; 2.5cm in diameter as cervix, plastic cup as vagina, foam rubber as posterior and anterior fornix and cotton plate as leukoplakia area. Totally, 34 participants performed LEEP and CKC on the training model with the guidance of mentors after theoretical lessons about transformation zone, indications and surgical technique of LEEP and CKC. Afterwards, a web-based survey was held to measure the effectivity of this surgical model and participants graded their learning and confidence levels.

Results: Educative level of the course on the basic surgical steps of LEEP and CKC procedures, and confidence level were evaluated with regard to the previous practice of LEEP and cold-knife conisation; and participants in each group had similar learning gains irrespective of previous practice. Despite a significantly higher pre-course confidence level for participants who have performed LEEP (p<0.001) and cold-knife conisation (p<0.001) previously, against the non-practitioner counterpart, post-course confidence levels were similar in each group (p=0.127, p=0.845, respectively). In both groups, participants had an increase in mean confidence scores and that was statistically significant for participants who have not performed the procedure previously (p=0.003, LEEP and p=0.002, CKC).

Conclusions: This surgical training model on LEEP and cold-knife conisation formed a good educative level for participants independent of their previous expertise.

Amaç: Loop electrosurgical excision procedure (LEEP) ve soğuk konizasyon eğitim modelinin jinekologların cerrahi eğitimi ve güven duygusu üzerine etkisini değerlendirmek.
Materyal ve metod: LEEP ve soğuk konizasyon uygulamalı eğitim modeli; 2.5cm çapta bir serviksi oluşturan sosis, vaginayı oluşturan plastik bardak, posterior ve anterior forniksi oluşturan köpük madde ve lökoplaki alanını sembolize eden pamuktan oluşmuştur. Transformasyon zonu, LEEP ve soğuk konizasyon endikasyonları ve cerrahi tekniği üzerine verilen teorik derslerden sonra, toplamda 34 katılımcı LEEP ve soğuk konizasyon eğitim modelini eğitmenler eşliğinde uygulamıştır. Uygulama sonrası, bu cerrahi modelin etkinliğini değerlendirmek üzere web tabanlı bir anket çalışması yapılmış olup, katılımcılar öğrenme düzeylerini ve güven duygusunu değerlendirmiştir.
Bulgular: Kursun LEEP ve soğuk konizasyona ait temel cerrahi basamaklar ve güven duygusu üzerine etkisi daha önce LEEP ve soğuk konizasyon yapmış olma durumuna göre değerlendirilmiştir. Her gruptaki katılımcılar daha önceki uygulamalarından bağımsız olarak benzer öğrenme değerleri elde etmiştir. Daha önce LEEP ve soğuk konizasyon yapmış katılımcılarda yapmamış olanlara göre anlamlı daha yüksek kurs öncesi güven duygusu olmasına rağmen (p<0.001), kurs sonrası güven duygusu her iki grup için de benzerdir. Her iki grup için de, ortalama güven duygusundaki artış daha önceden işlemi uygulamayan grup için istatistiksel olarak anlamlıdır (p=0.003, LEEP ve p=0.002, soğuk konizasyon).
Sonuç: LEEP ve soğuk konizasyon cerrahi eğitim modeli daha önceden işlemi uygulamadan bağımsız olarak katılımcılar için iyi bir eğitim seviyesi oluşturmuştur.
