The influence of hormonal status and socio-cultural determinants on postmenopausal sexual dysfunction.

Objective: To evaluate the hormonal and psycho-socio-cultural determinants that effect postmenopausal sexual dysfunction. Design: Case-control study. Setting: University hospital, Menopause clinics. Patients: 50 volunteers fit for inclusion criteria. Intervention: FSFI questionaire was applied to the participants after initial interview that preset questions evaluating psychosocio-cultural determinants were directed. Vaginal pH, total testosterone, and SHBG levels were measured in addition to routine menopause investigations. Results: FSFI scores were accordingly low in women who considered that they had a sexual dysfunction (50%) and, in those who felt menopausal transition had a negative impact in their sexual relations (52%) (p=0.00). While 6% of participants found sex after menopause as either shameful-unappropriate or unnecessary, other 17% stated that sex after menopause should continue as a duty of woman. 33% of the participants outlined that “motherhood” is the most important goal of their lives. Stepwise logistic regression analysis revealed a correlation between FSFI scores and FAI. Conclusion: FSD is prevalent in our country. Questioning the patient about the presence of sexual dysfunction may be sufficient
to disclose the problem. FAI is a more reliable parameter in selecting candidates for androgen therapy.
Menopoz sonrasında cinsel disfonksiyonun hormonal durum ve sosyo-kültürel faktörlerle etkileşimi.

Amaç: Menopoz sonrasında cinsel disfonksiyonu belirleyen hormonal ve psiko-sosyo-kültürel faktörlerin değerlendirilmesi. Planlama: Olgu kontrol çalışması. Ortam: Üniversite Hastanesi Menopoz polikliniği. Hastalar: Kabul kriterlerine uyan 50 gönüllü hasta. Girişim: Gönüllü hastalara psiko-sosyo-kültürel faktörlerin daha önce hazırlanmış sorularla değerlendirildiği bir ön görüşme
sonrasında FSFI anketi uygulanmıştır. Rutin menopoz muayene ve tetkiklerine ek olarak vajinal pH ölçümü ve total testosteron ve SHBG ölçülmüştür. Sonuç: Kendilerinde cinsel problem olduğunu düşünen (%50) ve menopozun cinsel hayatlarını olumsuz yönde etkilediğini düşünen kadınların(%52) FSFI skorları da uyumlu olarak düşük bulunmuştur(p=0.00). Çalışma grubunun %6’sı postmenopozal dönemde cinsel ilişkiyi ayıp-uygunsuz yada gereksiz olarak değerlendirirken, %17’si kadınlık görevi olarak cinsel hayatın devam etmesi gerektiğini düşünmektedirler. Hastalarımızın %33’ü hayattaki en önemli görevlerinin annelik olduğunu belirtmişlerdir. Stepwise
lojistik regresyon analizinde FSFI skorları SAİ ile ilişkilidir. Yorum: KCD’nun ülkemizde oldukça yaygın bir problemdir. Problemin varlığını ortaya koyabilmek için sadece hastanın kendi cinsel hayatı konusunda sorgulanması bile yeterli olabilir. Androjen tedavisine aday olabilecek hastaların seçiminde SAİ daha güvenilir bir parametredir.
