Penetran Keratoplasti Sonrası Sitomegalovirüs Endoteliti

Sitomegalovirüs (CMV) ilişkili endotelit, genellikle immün sistemi sağlam bireylerde, de-novo veya oküler cerrahiye sekonder gelişebilir. Keratoplasti sonrası gelişen CMV endoteliti, benzer klinik bulgular nedeniyle greft rejeksiyonu ile karışabilmektedir. Bu olgu sunumunda, penetran keratoplasti sonrası greft reddi gelişen ve bu nedenle immünsüpresif ajan kullanan bir olgu anlatılmıştır. Postoperatif dördüncü yılda; lokalize kornea ödemi, madeni para benzeri keratik presipitatlar ve göz içi basıncında artış görüldü. Aköz hümör örneğinden yapılan polimeraz zincir reaksiyonu analizinde, CMV-DNA saptandı. Valgansiklovir tedavisi başlanan olguda iki ay içinde semptomlar düzeldi. Keratoplasti sonrası kullanılan lokal veya sistemik immünsupresanların CMV reaktivasyonunu tetikleyebileceği akılda tutulmalıdır. Viral endotelit nedenleri arasında CMV de düşünülmeli, madeni para keratik presipitatların bulunduğu olgularda vakit kaybetmeden gansiklovir tedavisine başlanmalıdır.

Cytomegalovirus Endotheliitis after Penetrating Keratoplasty

Cytomegalovirus (CMV) related corneal endotheliitis is an inflammation of the corneal endothelium caused by CMV. It may occur de-novo or after ocular surgery in otherwise healthy individuals. In patients who have undergone keratoplasty, differential diagnosis of viral endothelitis and immune-related graft rejection is challenging due to the similar clinical findings. We report a case with penetrating keratoplasty and using local and systemic immunosuppressive agents because of previous graft rejection history. In the postoperative fourth year, ophthalmologic examination revealed localised corneal edema and coin-shaped keratic precipitates and increased intraocular pressure, consistent with viral endotheliitis. Polymerase chain reaction revealed CMV-DNA amplification in the aqueous humor sample. Valganciclovir treatment was started and the symptoms improved in two months. It should be kept in mind that local or systemic immunosuppressants used after keratoplasty may trigger CMV reactivation. Anti-CMV treatment should be initiated immediately in patients with coin-shaped keratic precipitates.
