Keratokonuslu Hastalarda Hibrit Kontakt Lenslerin Klinik Performansları ve Hasta Memnuniyeti

Gereç ve Yöntem: Çalışmaya KK tanısıyla HKL reçete edilen 68 hasta (35 kadın, 33 erkek) dahil edildi. Hastaların dosyalarından kornea topografi parametreleri, camla en iyi düzeltilmiş görme keskinliği (EİDGK), HKL deneme sayısı, reçete edilen HKL temel eğrisi ve HKL ile EİDGK not edildi. Hastalara elektronik posta veya WhatsApp yoluyla KL memnuniyet anketi gönderildi ve sonuçlar istatistiksel olarak IBM SPSS Statistics Version 22.0 ile değerlendirildi.
Bulgular: Çalışmaya yaş ortalaması 27,34±8 yıl (aralık 12-48 yaş) olan 68 hastanın 110 gözü dahil edildi. Amsler-Krumeich sınıflamasına göre gözlerin %35,5’i evre 1, %50,9’u evre 2 ve %13,5’sı evre 3-4 idi. Topografik ortalama K1 değeri 7,14±0,50 mm (aralık 5,72-8,30), K2 değeri 6,63±0,49 mm (aralık 5,07-7,84) ve Km değeri 6,89±0,48 mm(aralık 5,39-8,06) olarak bulundu. Ortalama KL deneme sayısı 1,59±0,82 (aralık 1-4) idi. Reçete edilen ortalama HKL temel eğrisi 6,84±0,50 mm (aralık 5,60-8,00) idi. Camla ortalama EİDGK 0,36±0,2 (aralık 0,05-0,8), HKL ile 0,80±0,14 (aralık 0,3-1,0) idi (p<0,001). Genel anket puanı 5 üzerinden 3,54, genel memnuniyet puanı 3,27, görme memnuniyet puanı 3,62, takma-çıkarma işleminde memnuniyet puanı 3,01, lens rahatlığında memnuniyet puanı 2,97 olarak bulundu.
Sonuç: Reçete edilen HKL temel eğri değerleri topografik Km değerine oldukça yakın bulunmuştur ve hastaların çoğunda ilk veya ikinci denemede başarılı uygulama yapılmıştır. Genel memnuniyet orta-iyi düzeyde bulunmuştur. Yumuşak KL kadar rahat olmaması, takma-çıkarma işlemlerinin zorluğu, lens ömrünün kısa ve maliyetinin yüksek olması ise dezavantajlarıdır.
Clinical Performance and Patient Satisfaction of Hybrid Contact Lenses in Patients with Keratoconus

Material and Method: 68 (35 female, 33 male) KC patients who were prescribed HCL were included in the study. Corneal topographic parameters, best corrected visual acuity (BCVA) with eyeglasses, the number of HCL trials, prescribed HCL base curve (BC) and visual acuity with HCL were recorded form hospital files. A CL satisfaction survey was sent to the electronic mails or via WhatsApp and the data was statistically analyzed using IBM SPSS Statistics Version 22.0.
Results: The study included 110 eyes of 68 patients with a mean age of 27.34±8 (range 12-48) years. According to the Amsler-Krumeich classification, 35.5% of the eyes were stage 1, 50.9% stage 2 and 13.5% stage 3 and 4. Mean K1, K2 and Kmean values were 7.14±0.50 mm (range 5,72-8,30), 6.63±0.49 mm (range 5,07-7,84) and 6.89±0.48 mm (range 5,39-8,06), respectively. The average number of lens trials was 1.59±0.82 (range 1-4). The mean BC of prescribed HCL was 6.84±0.50 mm (range 5,60-8,00). BCVA with glasses was 0.36±0.2 (range 0,05-0,8), and 0.80±0.14 (range 0,3-1,0) with HCL (p<0.0001). The overall survey score was 3.54 out of 5, the overall satisfaction score was 3.27, the average vision satisfaction score was 3.62, the average satisfaction score for lens insertion and removal was 3.01, and the average satisfaction score for lens comfort was 2.97.
Conclusion: Prescribed HCL BC are usually close to the topographic Kmean value and in most of the patients, successful fitting was done after the first or second CL trial. The overall satisfaction score was moderate to good and the disadvantages were low comfort compared to soft CL, difficulty in insertion and removal, short lens life and high cost.
