Latanoprost Tedavisine Dirençli Primer Açık Açılı Glokom Olgularında Bimatoprost veya Travoprost Monoterapisine Geçişlerde Oluşan Etkinliklerin Değerlendirilmesi

Amaç: Bu prospektif çalışma ile latanoprost tedavisine yanıtsız primer açık açılı glokom (PAAG) olgularında bimatoprost yada travoprost monoterapisine geçişle oluşan etkinliğin değerlendirilmesi amaçlandı.
Gereç ve Yöntem: Mart 2009 – Mart 2010 tarihleri arasında Gaziantep Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalında topikal latanoprost monoterapisi ile takipde olan 40 primer açık açılı glokom hastası alındı. Tedaviye dirençli yada daha fazla GİB (Göz içi basıncı) düşüşü istediğimiz 40 PAAG olgusunun latanoprost tedavisi kesildi. Hastaların 20’sinde bimatoprost, 20’sinde ise travoprost monoterapisi şeklinde ilaç değişimi yapıldı. Hastalar 6 ay süre ile takip edildi.
Sonuçlar: Latanaprost kullanımını bırakıp bimatoprosta geçiş yaptığımız birinci grupta başlangıca göre ortalama; 1. hafta sonunda 2.05 mmHg, 1. ayda 2.25 mmHg, 3. ayda 1.90 mmHg, 6. ayda ise 2.40 mmHg GİB düşüşü sağlanmıştır. Latanaprost kullanımını bırakıp travoprosta geçiş yaptığımız ikinci grupta başlangıca göre ortalama; 1. hafta sonunda 1.47 mmHg, 1. ayda 1.75 mmHg, 3. ayda 1.42 mmHg, 6. ayda ise 1.27 mmHg GİB düşüşü sağlanmıştır.Her iki ilaç grubunda da başlangıç santral korneal kalınlığı düzeyinde altı aylık takip sonunda istatistiksel olarak anlamlı bir değişiklik görülmemiştir.İki ilaç grubunda da en sık gözlenen yan etki oküler irritasyon ve konjonktiva hiperemisi olup bimatoprost kullanan olgularda, travoprost grubuna göre daha sık rastlandı.
Tartışma: Latanoprost tedavisine dirençli PAAG olgularında ikinci farklı bir ilaç ilavesinden önce, prostamid olan bimatoprost yada prostaglandin analoğu olan travoprost monoterapisine geçerek GİB kontrolünün sağlanabileceği görülmüştür.
Efficacy Of Monotherapy With Either Bimatoprost or Travoprost in Patients Who Are Resistant to Latanoprost Therapy in Primary Open Angle Glaucoma

Introduction: This prospective study aimed to evaluate the efficacy of monotherapy with either bimatoprost or travoprost in patients who are resistant to latanoprost therapy in primary open angle glaucoma (POAG).
Methods: During March 2009 to March 2010, 40 patients with POAG receiving therapy with topical latanoprost at Gaziantep University Hospital, Department of Ophthalmology were chosen to participate in this study. The decision to stop latanoprost therapy in these patients was made either due to the fact that they were resistant to therapy or further decrease in their intraocular pressure (IOP) needed to be obtained of the 40 patients, 20 were chosen to receive monotherapy with bimatoprost and 20 were chosen to receive monotherapy with travoprost. Patients were observed for 6 months with follow-up at 1 week, 1 month, 3 months and 6 months.
Results: For the group of patients receiving therapy with bimatoprost; the average decrease in IOP was measured at 1 week (2.05 mmHg), 1 month (2.25 mmHg), 3 months (1.90 mmHg) and 6 months (2.40 mmHg). For the group of patients receiving therapy with travoprost; the average decrease in IOP was measured at 1 week (1.47 mmHg), 1 month (1.75 mmHg), 3 months (1.42 mmHg) and 6 months (1.27 mmHg). At the end of 6 months, there were no significant changes observed in the mean deviation and central corneal thickness for both therapy groups. Conjunctival hyperemisave, ocular irritation was the most common side effect observed for both groups with the bimatoprost therapy group showing higher values than the travoprost group.
Discussion: For patients resistant to latanoprost therapy, prior to any adjuvant therapy, monotherapy with either bimatoprost, a prostamide, or travoprost, a prostaglandin analogue, has been shown to be effective at lowering IOP.
