The definition of cystoid macular degeneration in diabetic macular edema: an Oct-based single center studyGokcen Yalcin1, SENGÜL OZDEK1, Fatma Nur Baran Aksakal21Department Of Ophthalmology, Gazi University, Ankara, Turkey2Department Of Public Health, Gazi University, Ankara, Turkey Purpose: To describe cystoid macular degeneration(CMD) which has no clear definition in diabetic macular edema(DME) and to examine features in Optical coherence tomography(OCT) and Fundus Florescein Angiography(FFA).Methods: This study was created with OCT images of the patients who were followed in Gazi University between November 2011 and March 2015.259 eyes of 187 patients were included.The macular, peripheral ischemia and the type of edema was identified.Vitreomacular interface abnormalities, the integrity of the foveal contour, the internal reflectivity of the cysts and outer retinal layer defects were analysed from OCT images.The horizontal and vertical diameters of the largest cyst in the area of 1000μm from the center of the fovea were measured for the definition of CMD.Results: The horizontal, vertical diameter of the largest cyst were moderately positive corralated with the visual acuity (rs=0.349, r=0.419, respectively).Cases with a horizontal diameter of the largest cyst ≥450μm were grouped as CMD-H group.In this group to predict a visual acuity of ≤20/60 (sensitivity) was 58%.Cases with a horizontal diameter of the largest cyst <450 μm were grouped as CME-H group.Specificity in this group to predict a visual acuity ˃20/60 was 73%. Cases with a vertical diameter of the largest cyst ≥300μm were grouped as CMD-V group. In these eyes to predict a visual acuity ≤20/60 was 62%.Cases with a vertical diameter of the largest cyst <300 μm were grouped as CME-V group.In this group to predict a visual 45 acuity ˃20/60 was 69%.The groups of CME and CMD were formed according to these cut-off values.Conclusion: Cystoid macular degeneration in DME can be defined as those with a horizontal diameter of the largest cyst ≥450μm,vertical diameter of the largest cyst ≥300μm in the center of the fovea especially if it is associated with macular ischemia, outer retinal damage, loss of foveal contour and diffuse/mixed edema subtype.
