Katarakt Cerrahi Simülatör Cihazı Pratik Uygulama İle Korele mi?

Amaç:
Katarakt cerrahisi simülasyon cihazının pratik uygulama ile korelasyonunu ve cerrahi eğitime katkısını değerlendirmek.
Gereç ve Yöntem:
Kliniğimizde görev yapan 16 hekim, cerrahi tecrübelerine bakılarak 3 gruba ayrıldı. Cihazın tanıtımı yapıldıktan sonra katılımcılardan navigasyon, forseps, bimanuel uygulama, antitremor ve kapsüloreksis modülünü kapsayan aşamaları yapmaları istendi. Kapsüloreksis aşaması 5 kez tekrar edildi. Katılımcılardan kapsüloreksisi non-dominant el ile tekrar yapmaları istenerek, bu uygulama için de puanları belirlendi. Puanların cerrahi deneyim ve uygulama tekrarından ne kadar etkilendiği değerlendirildi. İstatistiksel değerlendirmede 0.05’in altındaki p değerleri anlamlı kabul edildi.
Bulgular:
Katılımcıların kapsüloreksis aşamasındaki puanlarının cerrahi deneyimleri ile korelasyon gösterdiği, dominant el ile yaptıkları kapsüloreksisin non-dominant ele kıyasla daha başarılı olduğu saptandı (p=0.004). Tekrarlayan uygulamalar sonrasında kapsüloreksis başarısının arttığı görüldü (p=0.001).
Sonuç:
Katarakt cerrahi simülatör cihazı cerrahi deneyim ile korele sonuçlar vermektedir. Tekrarlayan uygulamalarda katılımcının performans artışı, cerrahi beceriye katkı sağladığını göstermektedir.

Does Cataract Surgery Simulator Device Work With Practice?

Objective:
To evaluate the correlation of cataract surgical simulator and real-life surgical applications and its contribution to surgical training.
Material and Methods:
Three groups were established from 16 doctors in our department, regarding their surgical experience. After introduction of the device, the participants were asked to perform the stages including navigation, forceps, bimanual practice, anti-tremor and capsulorhexis and their points were recorded. Capsulorhexis stage was repeated 5 times. Participants were also asked to perform capsulorhexis again with their non-dominant hand and their points were determined for this practice. The influence of repetition and surgical experience on the recorded points was evaluated. The p values below 0.05 were considered statistically significant.
Results:
There was correlation between the surgical experience of the participants and their achieved points in capsulorhexis module. Their dominant hand was more successful than the non dominant hand in capsulorhexis (p=0.004). Success in capsulorhexis increased up on repetition (p=0.001).
Conclusion:
Cataract surgery simulation device gives correlated results with surgical experience. The increase of performance upon repeated practice, points that it promotes surgical training.
