Akut maküler nöroretinopati ile seyreden Purtscher retinopatisi
Berrak Şekeryapan GedizSağlık Bilimleri Üniversitesi Ankara Ulucanlar Göz Eğitim Ve Araştırma HastanesiPurtscher retinopatisi ve akut maküler nöroretinopati nadir rastlanan klinik durumlar olup, her ikisinin de patogenezinde iskemi suçlanmaktadır. Bu yazıda göğüs travması sonrası görsel şikayetleri olan bir olguda izlenen Purtscher retinopatisi ve akut maküler nöroretinopati birlikteliği ilk kez sunulmaktadır. Optik koherens tomografide dış retinal katlarda hasar, optik koherens tomografi anjiografide ise yüzeyel ve derin kapiller pleksus ile birlikte koryokapillariste hipoperfüzyon alanları izlenmiştir. Bu yazıda klinik tanısı güç olsa da optik koherens tomografi ve optik koherens tomografi anjiografideki karakteristik bulgularıyla tanı konulması kolaylaşan akut maküler nöroretinopatinin travma sonrası görme kaybı ile başvuran hastada akılda bulundurulması gereken bir durum olduğu vurgulanmaktadır.

In this report, the association of Purtscher retinopathy and acute macular neuroretinopathy in a patient with visual complaints after chest trauma as the first report of this association in the literature was described. Optical coherence tomography scans demonstrated outer retinal defects. Optical coherence tomography angiography illustrated superficial and deep capillary plexuses hypoperfusion with choriocapillaris flow void. In this report, it is emphasized that acute macular neuroretinopathy is a clinical condition that should be kept in mind in patients presenting with post-traumatic vision loss. Although its clinical diagnosis is difficult, characteristic optic coherence tomography and optic coherence tomography angiography findings are very useful in diagnosing.
