Buckle Handle in the Orbit

On examination he was conscious, cooperated and oriented. Neurological examination was normal. The ophthalmologic examination showed that his visual acuity was 20/20 in the right eye and counting fingers from 2 feet in the left eye. Biomicroscopic examination was normal in the right eye. The left globe was normal other than edematous eyelids and subconjunctival hemorrhage. There was a large premacular hemorrhage left fundus examination. X-ray examination of the cranium and orbital computerized tomography showed that the tip of the hook had entered into the retroorbital space without penetrating the globe or any bony tissue. The optic nerve was found to be unaffected. Intravenous cephazoline sodium 1000 mg two times daily and gentamycin 80 mg two times daily were started with tetanus vaccination. The patient was taken to the operation room for removal of the foreign body and detection of any associated penetrating injury. During operation, the left globe was found totally intact. On the postoperative fifth day, the preretinal hemorrhage opened into the vitreous cavity and the patient’s visual acuity decreased to hand movement and light perception level. The intravitreal hemorrhage persisted in his follow-up, so pars plana vitrectomy was performed. 3 months after the operation his left eye the best corrected visual acuity was 20/60 and remained in this level.

Başvuru anında hastanın nörolojik ve cerrahi konsültasyonları ile hayati tehlikesinin olmadığı tespit edildi. Görme keskinliği sağ gözde 1,0 sol gözde iki metreden parmak sayma düzeyinde idi. Sol gözde kapak ödemi ve subkonjonktival hemoraji mevcut idi. Yabancı cisim temporalde kapak aralığından sarkar tarzda yerleşmişti. Göz dibi incelemesinde geniş bir preretinal hemoraji saptandı. Sağ göz muayenesi ise normaldi. Direkt grafi ve kranial bilgisayarlı tomografide büyük yabancı cismin uç kısmının globu ve kemik dokuyu zedelemeksizin glob arkasına ulaştığı gözlendi. Hasta yabancı cismin çıkarılması ve eksplorasyon için ameliyathaneye alındı. Eksplorasyon sırasında perforasyon açısından sklera dikkatlice incelendi ve yabancı cisim başarıyla çıkarıldı. Hastaya intravenöz sefazolin sodyum 1gr 2x1, intravenöz gentamisin 80 mg 2x1 ve tetanoz aşısı uygulandı. Postoperatif beşinci günde preretinal kanama vitreye açıldı ve oluşan vitre içi kanama hastanın görme keskinliğini el hareketi ve ışık projeksiyonu düzeyine düşürdü. Vitre içi kanama gerilemeyince hastaya pars plana vitrektomi operasyonu yapıldı. Ameliyattan 3 ay sonra hastanın düzeltilmiş en iyi görme keskinliği 0,3 idi ve bu düzeyde kaldı.
