Bilateral Multifocal Intraocular Lens Implantation on an Postoperative Hyperopic Patient

PURPOSE: Presentation of our bilateral multifocal intraocular lens (IOL) implantation results on an bilateral postoperative hyperopic laser in situ keratomileusis (LASIK) patient.
MATERIAL-METHODS: In April 2007, a female, 10 year- postoperative bilateral hyperopic LASIK patient was implanted by the same surgeon with bilateral multifocal IOL’s following a facoemulsification operation, because she had developed cataract. Patient’s preoperative and postoperative visual acuities (distance, near and intermediate on both Snellen and Jaeger charts), corneal topography, keratometry values and postoperative contrast sensitivity results were followed up. Power of the IOL was calculated according to clinical narrative method. Patient was also asked questions regarding her quality of life, light reflections and personal satisfaction.
RESULTS: 70 year old female subject had received hyperopic LASIK treatment 10 years ago (OD: +3.75 D; OS: +3.50 -0.50 90 D). After cataract has developed, in April 2007, patient received bilateral multifocal IOL implantation following facoemulsification surgery. Right eye received a refractive IOL, while left eye received an aspheric diffractive IOL. At 2 months postoperative, uncorrected visual acuities were as follows; distant 0.8 and 1.0, near J3 and J1, intermediate J5 and J3. Postoperative best corrected visual acuities were as follows; distant 1.0 and 1.0, near J1 and J1, intermediate J2 and J1. At 2 months, postoperative refraction values were -0.75 D for the right eye and +0.00 for the left eye. Contrast sensitivity tests were conducted at 4th, 6th months and 1 year post–op and improvement was observed. Patient noted the glare problems were diminishing by day. Overall satisfaction was rated as ‘good’.
CONCLUSION: Our bilateral hyperopic LASIK patient was satisfied following the bilateral multifocal IOL implantation. Although the result is encouraging, further studies on more subjects are needed for proving the effectiveness and safety of the technique.
Hipermetropik LASİK uygulanmış bir olguda bilateral multifokal göziçi lensi implantasyonu

AMAÇ: Bilateral hipermetropik laser in situ keratomileusis (LASİK) uygulanmış bir olguda bilateral multifokal göz içi lensi (GİL) implantasyonu sonucumuzu sunmak.
GEREÇ-YÖNTEM: On yıl önce bilateral hipermetropik LASİK uygulanmış bayan hastaya katarakt gelişmesi nedeniyle nisan 2007’de aynı cerrah tarafından bilateral fakoemulsifikasyon (FAKO) ile birlikte multifokal GİL implantasyonu yapıldı. Olgunun preoperatif ve postoperatif uzak, yakın ve orta mesafedeki görme keskinlikleri (Snellen ve Jaeger eşelleri), korneal topografisi, keratometrik değerleri ve postoperatif kontrast duyarlılık sonuçları incelendi. GİL gücü klinik öykü yöntemine göre hesaplandı. Olguya ayrıca yaşam kalitesini, ışık yansımalarını ve kişisel memnuniyetini irdeleyen sorular soruldu.
BULGULAR: Yetmiş yaşındaki bayan olgumuza 10 yıl önce hipermetropik LASİK (OD: +3.75 D; OS: +3.50 -0.50 90 D) yapılmıştı. Katarakt gelişmesi nedeniyle hastaya bilateral FAKO ile birlikte multifokal GİL implantasyonu yapıldı. Sağ göze refraktif lens, sol göze ise asferik difraktif lens implante edildi. Postoperatif 2. aydaki tashihsiz görme keskinlikleri uzak mesafede 0.8 ve 1.0 iken, yakın mesafede J3 ve J1, orta mesafede ise J5 ve J3 olarak bulundu. Olgunun tashihli görme keskinlikleri ise uzak mesafede 1.0 ve 1.0; yakın mesafede J1 ve J1; orta mesafede J2 ve J1 idi. Postoperatif 2. aydaki refraksiyon değerleri sağda -0.75, solda +0.00 idi ve bu değerler takip süresince stabil kaldı. Postoperatif 4. ayda, 6. ayda ve 1. yılda kontrast duyarlılık testlerinde zaman içinde iyileşme gözlendi. Hasta, başlangıçta kamaşma sorunlarının olmasına rağmen, bunların her geçen gün azaldığını belirtti. Hasta memnuniyeti ‘iyi’ olarak değerlendirildi.
SONUÇLAR: Bizim bu bilateral hipermetropik LASİK'li olgumuzda bilateral multifokal GİL implantasyonu ile hasta memnuniyeti sağlanmış ve teknik cesaret verici bulunmuştur. Ancak yöntemin etkinlik ve güvenilirliğinin saptanması için olgu sayısının artırılmasına ihtiyaç vardır.
