Conjunctivolimbal autograft attaching with fibrin glue in pterygium surgery

Methods: Twenty-six eyes of 26 patients with nasal pterygium who were treated with pterygium excision with superior conjunctivolimbal transplantation and fibrin glue were reviewed. Surgical durations were recorded. Patients were followed up postoperatively on days 1,3,7,14 and then at months 1,2 and 3. Subjective symptoms of patients were recorded at every follow-up examinations. Postoperative complications and recurrence rate was evaluated.
Results: The mean patient age was 57.80±9.58 (range 39-78) years. The mean follow-up was 20.84±6.50 weeks. The mean surgery time was 12.84±1.73 minutes. The subjective symptoms (pain, foreign body sensation, epiphora, irritation) disappeared in 18 of 26 eyes (%69) in one week after surgery and all discomforts reduced within two weks after surgery in all patients. One patient (%3.8) had partial graft dehiscence; it was reattached with fibrin glue. There were no cases of pterygium recurrence or complications due to the fibrin glue.
Conclusions: Fibrin glue is a safe and effective method for attaching conjunctival autografts. The use of fibrin glue results in shorter operating time and less postoperative discomfort.
Pterjium cerrahisinde fibrin yapıştırıcısı ile konjonktivalimbal otogreft uygulaması

Yöntem: Nasal pterjiumu olan ve pterjium eksizyonu yapılarak; fibrin yapıştırıcısı ile üst konjonktivalimbal otogreft transplantasyonu yapılan 26 hastanın 26 gözü incelendi. Cerrahi süreleri kaydedildi. Hastalar ameliyat sonrası 1.,3.,7., 14 günler ve 1.,2. ve 3. aylarda kontrole çağrıldı. Her kontrolde hastaların subjektif şikayetleri kaydedildi. Ameliyat sonrası komplikasyonlar ve nüks oranları değerlendirildi.
Bulgular: Hastaların ortalama yaşı 57.80±9.58 (39-78)di. Ortalama takip süresi 20.84±6.50 hafta idi. Ortalama cerrahi süresi 12.84±1.73 dakikaydı. 26 gözün 18’inde (%69) ameliyat sonrası ilk haftada subjektif semptomlar (ağrı, yabancı cisim hissi, epifora, irritasyon) ortadan kalktı. Hastaların tüm şikayetleri ise ameliyat sonrası 2 hafta içinde azaldı. Bir hastada(%3.8) greftte kısmi katlanma görüldü, fibrin yapıştırıcısı ile tekrar yapıştırıldı Hiçbir hastada takip süresince pterjiumda tekrarlama veya fibrin yapıştırıcısına bağlı komplikasyon görülmedi.
Sonuç: Konjonktival otogreftlerin yerleştirilmesinde fibrin yapıştırıcısı etkili ve güvenli bir yöntemdir. Fibrin yapıştırıcısının kullanımı cerrahi süresini kısaltmakta; cerrahi sonrası şikayetleri azalmaktadır.
