Results of Surgery in Patients with type 1 Duane Retraction Syndrome

Objective: . To assess results of surgery techniques used in Duane retraction syndrome(DRS) type 1
Material & Methods: 194 DRS cases tracked in department of strabismus were retrospectively investigated. It was assesed deviation on pre and post-operative primer position, AHP,abduction deficency glob retraction and up-down shoot of 58 cases with esotropic DRS type 1 performed surgeries. Mean follow up period was 22.4± 4.6 (1- 13 years) months.
Results: In group1 cases underwent recession of uni or bilateral MR muscles,postoperative 1st month improvment rates for distance deviation, near deviation and AHP were %68,6, %73,9 and %50 respectivetly. These rates were %69.3, %64.7 and %57.1 respectivetly in group 2 underwent VRT surgery. In group 3, which we performed Y split (with or without LR/MR recession) surgery improvment rates for distance and near deviatiaon were % 63.6, % 63.9 and % 50 was determined for AHP. Improvment in globe retraction were % 60 in group 1 and % 66.6 in group 3.
Conclusion: Medial rectus muscle recession was more effective for recovery of deviation and AHP on DRS type 1. VRT was found to be more effective for recovery of abduction deficiency. Surgery of y split has an important effect on recovery of globe retraction and up/down shoot.

Amaç: Tip 1 Duane retraksiyon sendromu(DRS) olgularında uygulanan cerrahi tekniklerin sonuçlarını değerlendirmek.
Gereç ve yöntem: Şaşılık bölümünde takip edilen 194 DRS’lu olgu retrospektif olarak incelendi. Cerrahi uygulanan ezotropik tip 1 DRS’lu 58 olgu; operasyon öncesi ve sonrası primer pozisyondaki kayma miktarı, anormal baş pozisyonu (ABP), abduksiyon kısıtlılığı, glob retraksiyonu ve aşağı/yukarı atımları açısından değerlendirildi. Olgular uygulanan cerrahi tipine göre 3 gruba ayrıldı. Olguların ortalama takip süresi 22.4 ± 4.6 ay (1- 13 yıl ) idi.
Bulgular: Bir veya her iki MR’lara geriletme uygulanan grup1’de, postoperatif 1. ayda uzak kaymanın %68.6, yakın kaymanın %73.9 ve ABP’nun %50 oranlarında düzeldiği, VRT cerrahisi uygulan grup 2 de; bu oranlar sırasıyla % 69.3, %64.7 ve %57.1 olarak saptanmıştır. Y split (LR/MR geriletmesi ile beraber olan/olmayan) cerrahisi uygulanan grup 3’de ise uzak kaymanın % 63.6, yakın kaymanın %63.9 ve ABP’nun %50 oranında düzeldiği saptandı. Glob retraksiyonundaki düzelme grup1’de %60, grup3’te %66.6 olarak bulunmuştur.
Sonuç: Tip 1 DRS’nda medial rektus kaslarının geriletilmesi, primer pozisyondaki kayma ve ABP’unun giderilmesinde etkilidir. VRT ise özellikle abduksiyon kısıtlılığın giderilmesinde etkilidir. Y split cerrahisi, glob retraksiyonunun ve yukarı/aşağı atımın düzeltilmesinde önemli etkiye sahiptir.
