The results of surgical treatment for cyclotropia

Methods: Twentytwo eyes of 17 patients with cyclotropia were included in the study. All the cases were examined at preoperative and postoperative period. We measured vertical and horizontal deviations with prism cover test, eye movements, oblique muscle functions, the degree of cyclotropia with double Maddox rod and fundus photographs. Surgical techniques were anterior transposition of the inferior oblique muscle, inferior oblique recession, superior oblique tenotomy and superior oblique tucking.
Results: There were 6 males and 11 females. The mean age of the patients was 17.4±9.7 years (6 to 34 years) and the mean follow- up time was 6.9±3.1 months (3 to 12 months). Eight patients (47.1%) presented superior oblique palsy. Seven patients had a concomitant horizontal deviation; 29.4% of the patients had esotropia and 11.7% of the patients had exotropia. Eight eyes underwent anterior transposition of the inferior oblique muscle, 8 eyes underwent inferior oblique recession, 2 eyes underwent superior oblique tuck and 4 eyes underwent superior oblique tenotomy. Anterior transposition of the inferior oblique muscle and inferior oblique recession were found as effective procedures for the treatment of vertical deviations and cyclotropoia but there was no statistically significant difference between two procedures.
Conclusion: Anterior transposition of the inferior oblique muscle and inferior oblique recession were found as effective procedures for the treatment of vertical deviations and cyclotropia. Anterior transposition of the inferior oblique muscle was found to be more effective than inferior oblique recession in the treatment of the inferior oblique over action.
Siklovertikal kaymalarda cerrahi sonuçlar

Gereç ve yöntem: Çalışmaya torsiyonel kayması olan 17 hastanın 22 gözü dahil edilmiştir. Hastaların preoperatif ve postoperatif prizma örtme testi ile vertikal ve horizontal kayma miktarları, göz hareketleri, oblik kas fonksiyonları, çift Maddox çubuğu ve fundus fotoğrafı ile torsiyon miktarları ölçülmüştür. Cerrahi yöntem olarak alt oblik anterior transpozisyonu, alt oblik geriletme, üst oblik tenotomi, üst oblik katlama uygulanmıştır.
Bulgular: Hastaların 6’sı erkek, 11’i kadın, ortalama yaş 17.4±9.7 y (6-34 y), ortalama takip süresi 6.9±3.1 ay (3-12 ay) idi. Hastaların 8’inde (%47.1) üst oblik felci mevcut idi. Hastaların 7’sinde beraberinde horizontal kayma mevcut olup %29.4’ünde ezotropya, %11.7’sinde ekzotropya mevcuttu. Sekiz göze alt oblik anterior transpozisyonu, 8 göze alt oblik geriletme, 2 göze üst oblik katlama, 4 göze üst oblik tenotomi uygulanmıştır. Alt oblik geriletmesi ve alt oblik anterior transpozisyonunun vertikal kayma ve torsiyonu azaltmadaki etkinlikleri istatistiksel olarak anlamlı bulunmuştur ancak iki grup arasında anlamlı fark saptanmamıştır. Alt oblik aşırı fonksiyonunu düzeltmede transpozisyon grubunun geriletmeye göre anlamlı olarak daha etkili olduğu görülmüştür.
Tartışma: Alt oblik geriletme ve alt oblik anterior transpozisyonu, siklovertikal kaymalarda vertikal kayma ve torsiyonu düzeltmede etkin cerrahi yöntemlerdir. Alt oblik aşırı fonksiyonunu düzeltmede transpozisyon, alt oblik geriletmeye göre daha etkin bulunmuştur.
