The effects of cataract surgery on anterior segment parameters in primary open-angle glaucoma and pseudoexfoliation glaucoma in the early post-operative period

Purpose: Our purpose was to compare the effect of cataract surgery on anterior segment parameters measured by optical biometry in primary open-angle glaucoma (POAG) and pseudoexfoliation glaucoma (PXG).
Material and methods: Twenty-five eyes of 25 cases with POAG and 29 eyes of 29 cases with PXG, those had uncomplicated phacoemulsification and posterior chamber intraocular lens implantation surgery were included to our prospective study. Central corneal thickness (CCT), anterior chamber depth (ACD) and axial length (AL) measurements were done by optical biometer preoperatively and at the first postoperative month. The pre and postoperative values of intra ocular pressure (IOP) and the anterior segment parameters and the differences between POAG and PXG were compared statistically by paired t, independent t and chi-square tests.
Results: The mean values of preoperative CCT (p=0,042) and ACD (p=0,012) were significantly lower in PXG than POAG group. In PXG group, the IOP decreased (p=0,001) but the CCT (p=0,03) and ACD (p=0,001) increased significantly postoperatively. But AL didn’t change significantly. In POAG group, the IOP decreased (p=0,01) but ACD (p=0,004) increased significantly postoperatively. But AL and CCT didn’t change significantly. There were no significant differences between the changes of preoperative and postoperative IOP (p=0,76), AL (p=0,44) and CCT (p=0,52) values between the two groups. But the postoperative increase in ACD was more in PXG (p=0,03).
Conclusions: Cataract surgery may cause some changes in IOP and anterior segment parameters like ACD and CCT postoperatively in eyes with POAG and PXG and these changes may differ in eyes with PXG and POAG.
Primer açık açılı glokom ve psödoeksfoliatif glokomlu olgularda, katarakt cerrahisinin ön segment parametreleri üzerine olan erken dönem etkileri

Amaç: Primer açık açılı glokom (PAAG) ve psödoeksfoliatif glokomlu (PEG) olgularda, katarakt cerrahisinin, optik biyometri ile ölçülen ön segment parametrelerine olan erken dönem etkilerinin kıyaslanması amaçlandı.
Gereç ve yöntem: Komplikasyonsuz fakoemülsifikasyon ve arka kamara göz içi lens (GİL) implantasyonu implantasyonu yapılan PAAG’lu 25 hastanın 25 gözü ve PEG’lu 29 hastanın 29 gözü prospektif çalışmamıza dahil edildi. Merkezi korneal kalınlık (MKK), ön kamara derinliği (ÖKD) ve aksiyel uzunluk (AU) ölçüleri, cerrahi öncesinde ve cerrahi sonrası 1. ayda optik biyometri ile ölçüldü. Cerrahi öncesi ve sonrası 1. aydaki göz içi basıncı (GİB) ve ön segment parametrelerinin değişimleri ve bu değişimlerin PAAG ve PEG’lu hastalar arasındaki farkları t test, bağımsız değişkenli t test ve ki-kare testleri ile istatistiksel olarak analiz edildi.
Bulgular: PEG’lu gözlerde PAAG’lu gözlere oranla cerrahi öncesi MKK (p=0,042) ve ÖKD (p=0,012) değerleri anlamlı ölçüde düşüktü. PEG’lu gözlerde cerrahi sonrası 1. ayda GİB anlamlı ölçüde düşerken (p=0,001), MKK (p=0,03) ve ÖKD (p=0,001) değerlerinde artış gözlendi. AU değerlerinde ise anlamlı fark görülmedi. PAAG’lu gözlerde cerrahi sonrası 1. ayda GİB anlamlı ölçüde düşerken (p=0,01), ÖKD (p=0,004) değerlerinde artış izlendi. MKK ve AU değerlerinde ise anlamlı farklara rastlanmadı. PEG ve PAAG’lu gözler arasında, cerrahiye bağlı ön segment değişimlerinin farkları incelendiğinde ise, GİB (p=0,76), AU (p=0,44) ve MKK (p=0,52) değişimleri arasında anlamlı farklara rastlanmazken, cerrahi sonrası ÖKD artışının PEG’lu gözlerde daha fazla olduğu izlendi (p=0,03).
Sonuç: Katarakt cerrahisi PAAG ve PEG’lu gözlerde GİB ve MKK ve ÖKD gibi ön segment parametrelerinde anlamlı değişikliklere sebep olabilmekte, bu değişimler de PAAG ve PEG’lu gözler arasında farklılıklar gösterebilmektedir.
