Comparison of Intraocular Pressure Measurements in Healthy Pediatric Patients using Three Types of Tonometers

Materials and Methods: Seventy-eight eyes of 78 patients under the age of 18 who underwent a routine ophthalmologic examination were included in the study. IOP was measured using Tono-Pen (TP) tonometry, Goldmann Applanation Tonometry (GAT), and Non-Contact Tonometry (NCT), respectively. IOP was adjusted based on the central corneal thickness (CCT). Patients with any ocular disorders other than a limited refractive error were excluded from the study.
Results: The study consisted of 46 girls and 32 boys. The mean age was 12.6±2.7 (range: 5–17). The mean CCT was 559.3±35.3. The mean refractive error was - 0.50±1.70. The mean level of visual acuity was 0.98±0.1 (range: 0.3–1.0) using the Snellen chart. Significant differences were found between the measurement results of each of three methods. Mean IOP was 12.1±2.2 mmHg for TP, 15.7±2.5 mmHg for GAT, and 17.1±3.1 mmHg for NCT. The correlations between measurement methods revealed that the highest correlation was between NCT and GAT (P < 0.001, r = 0.670). The second highest correlation was between NCT and TP (P < 0.001, r = 0.477). The lowest correlation was between GAT and TP (P < 0.001, r = 0.403). A positive correlation was found between CCT and each IOP measurement method.
Conclusion: In pediatric patients, the TP and NCT measurements were found to have positive correlations with the GAT measurements. Because the TP measurements were lower than the GAT measurements and the NCT measurements were higher than the GAT measurements, patient follow-ups, treatment strategies, and surgery plans have to be organized taking these differences into consideration.
Sağlıklı Pediatrik Hastalarda Üç Farklı Tonometre Modeli ile Yapılan Göz İçi Basıncı Ölçümlerinin Karşılaştırılması

Yöntem – gereçler: Çalışmaya rutin poliklinik muayenesi için başvuran, 18 yaş altı 78 bireyin 78 gözü dahil edildi. GİB sırasıyla Tono- Pen (TP), Goldmann Aplanasyon Tonometresi (GAT) ve Non-Kontakt Tonometre (NKT) yardımı ile ölçüldü. Santral kornea kalınlıklarına (SKK) göre düzeltilmiş GİB hesaplandı. Belirlenmiş sınırlardaki refraksiyon kusuru dışında başka bir oküler hastalığı olan bireyler çalışmaya dahil edilmedi.
Bulgular: Çalışmaya dahil edilen bireylerden 46’sı kız, 32’si erkekti. Hastaların yaş ortalaması 12.6±2.7 (5-17 yaş arası) idi. Ortalama SKK 559.3±35.3 idi. Ortalama refraksiyon değerleri -0.50±1.70 idi. Görme keskinliği Snellen eşeline göre ortalama 0.98±0.1 (aralık 0.3 – 1.0) seviyesindeydi. Her 3 ölçüm yöntemi arasında anlamlı fark mevcuttu. (P < 0.05) 78 gözde ortalama GİB, TP ile 12.1±2.2 mmHg, GAT ile 15.7±2.5 mmHg ve NKT ile 17.1±3.1 mmHg olarak ölçüldü. Ölçüm yöntemlerinin birbiri ile korelasyonu incelendiğinde en yüksek pozitif korelasyon NKT ile GAT arasında tespit edildi. ( P < 0.001, r = 0.670) Bunu sırasıyla NKT ile TP ( P <0.001, r = 0.477) ve GAT ile TP ( P <0.001, r = 0.403) arasındaki korelasyon izledi. Yapılan incelemede SKK ile tüm ölçüm yöntemleri arasında pozitif korelasyon saptandı.
Sonuç: Pediatrik hastalarda TP ve NKT ile yapılan ölçümler, GAT ölçümleri ile pozitif korelasyon göstermekteydi. TP’nin GAT’ye göre daha düşük, NKT’nin ise daha yüksek değerler verdiği öngörüldüğünde hasta takiplerinde, tedavi düzenlemesi ve ameliyat kararlarında bu farklılıklar göz önünde bulundurularak hareket edilmesi önerilebilir.
