Comparison of Descemet Stripping Automated Endothelial Keratoplasty and Penetrating Keratoplasty in a Patient with Fuchs Endothelial Dystrophy

Case report: A 74-year-old female patient with diagnosis of FED underwent uncomplicated cataract surgeries of both eyes, but developed bullous keratopathy eventually. Right eye underwent DSAEK and left eye underwent PK. After 3 years of follow-up, right eye had an uncorrected visual acuity (UCVA) of 20/40 and best corrected visual acuity (BCVA) of 20/20 with spectacle correction (+2.25(-0.50x20°). Graft was clear with endothelial cell density (ECD) of 1532 cells/mm2 (donor ECD was 3215 cells/mm2) and central corneal thickness (CCT) was 674 μm. After 1 year of follow-up, left eye had UCVA of 20/200, spectacle corrected (-3.50x100°) visual acuity of 20/125 and BCVA of 20/40 with rigid contact lens correction. Graft was also clear with ECD of 1785 cells/mm2 (donor ECD was 3100 cells/mm2) and CCT was 610 μm.
Conclusion: This case report represents the advantages and disadvantages of two surgical techniques (PK and DASEK) applied on fellow eyes of a patient with FED.
Fuchs Endotelyal Distrofili bir Hastada Desme Soymalı Otomatik Endotelyal Keratoplasti ve Penetran Keratoplasti Karşılaştırması

Olgu Sunumu: Fuchs endotelyal distrofisi tanısı ile 74 yaşındaki bayan hastanın her iki gözüne komplikasyonsuz katarakt ameliyatı uygulandı, fakat neticede büllöz keratopati gelişti. Sağ göze DSOEK ve sol göze PK yapıldı. Takip edilen 3 yıldan sonra, sağ gözde düzeltilmemiş görme keskinliği (DGK) 20/40 ve düzeltilmiş en iyi görme keskinliği (DEGK), (+2.25(-0.50x20°) gözlük ile 20/20 idi. Greft, 1532 hücre/mm2 endotel hücre yoğunluğu (EHY) ile birlikte saydamdı (donör EHY 3215 hücre/mm2 idi) ve santral kornea kalınlığı (SKK) 674 μm idi. Takip edilen 1 yıldan sonra, sol gözde DGK 20/200 ve (-3.50x100°) gözlük ile görme keskinliği 20/125 idi ve DEGK sert kontakt lens ile 20/40 idi. Greft te 1785 hücre/mm2 EHY ile birlikte saydamdı (donör EHY 3100 hücre/mm2 idi) ve SKK 610 μm idi.
Tartışma: Bu olgu sunumu, FED sebebiyle aynı hastanın farklı gözlerine uygulanan iki cerrahi tekniğin (PK ve DSOEK) avantaj ve dezavantajlarını göstermektedir.
