High Resolution Optical Coherence Tomography in Late Stage Familial Foveal Retinoschisis

The patient had 5 brothers, all reported to have similar retinal problems. His best-corrected visual acuity was 4/10 in both eyes. Fundus examination showed retinoschisis in the lower temporal quadrants in both eyes, pigmented laser scars in the same area in the right eye, and a dull foveal reflex was observed in both eyes. The foveal sections examined with high resolution optical coherence tomography (OCT) demonstrated atrophy in the fovea bilaterally. OCT sections did not show any accumulation of intraretinal or subretinal fluid or any cystic cavities; yet, thinning was observed especially on the superficial retinal layers. Structures under the outer plexiform layer were observed as normal. Even though these OCT findings are not merely diagnostic as in early stage familial foveal retinoschisis, there is no doubt that they still facilitate the diagnosis.

Hakan Özdemir, Murat Karaçorlu, Serra Arf, Fevzi Şentürkİstanbul Retina Enstitüsü, İstanbulÇocuklukdan beri her iki gözünde görme bulanıklığı olan 35 yaşındaki erkek hastaya 14 yaşında iken foveaskizis teşhisi konulmuştu. Hastanın 5 erkek kardeşi vardı ve bunların hepsinde benzer bir retina problemi tespit edildiği söylenmişti. Her iki gözde tashihle görme keskinliği 4/10 seviyesindeydi. Fundus muayenesinde her iki gözde alt temporal kadranlarda retinoskizis, sağ gözde aynı bölgede pigmente laser skarları ve her iki gözde silik fovea reflesi izlendi. Yüksek çözünürlüklü optik koherens tomografi (OCT) incelemesinde foveal kesitlerde her iki gözde foveanın atrofik olduğu görüldü. OCT kesitlerinde intraretinal ya da subretinal sıvı birikimi ya da kistik boşluklar gözlenmezken, retina incelmesinin özellikle yüzeyel tabakalarda görüldüğü tespit edildi. Dış pleksiform tabakanın altındaki yapılar normal görünümdeydi. OCT bulguları familial foveal retinoskizisin erken dönemlerindeki gibi tanı koydurucu olmasa da, hastalığın geç dönemlerinde teşhise yardımcı olacağına şüphe yoktur.
