The efficacy of intravitreal bevacizumab in vitreous hemorrhage of diabetic subjects

Objectives: To evaluate the efficacy of intravitreal bevacizumab in the resolution of vitreous hemorrhage (VH) secondary to proliferative diabetic retinopathy (PDR).
Material and Methods: Seventy eyes of 70 patients (43 male, mean age 55.6±12.2 years) diagnosed to have VH secondary to PDR were evaluated retrospectively. Demographic characteristics of the patients, baseline and final clinical results, and the interventions the patients were subject to were recorded. The patients who received IVB injections (Group 1, n = 29) were compared to those who did not receive injections (Group 2, n = 41) for VH clearance time and rate of surgery.
Results: The mean follow-up time was 14.5±6.1 months in Group 1 and 18.4±9.6 months in Group 2 (p=0.185). The mean visual acuity was similar between the groups at baseline and at the last visit (for all p>0.05). Panretinal photocoagulation could be applied in 86% of subjects in Group 1 and in 58 % in Group 2 within one month (p=0.016). VH clearance time was not different between the groups (2.3±2.1 months in Group 1 and 3.4±2.6 months in Group 2, p=0.146). The number of subjects requiring surgery was 7 (24%) in Group 1 and 20 (%48.8) in Group 2 (p=0.048).
Conclusion: Intravitreal bevacizumab was found efficient in cases with VH secondary to PDR in terms of reducing the need for surgery and increasing the rate of subjects to whom panretinal photocoagulation could be applied in the early period, although no impact was present on final visual acuity.

Diyabetik olgularda intravitreal bevacizumab’ın vitreus hemorajisinde etkinliği

Amaç: Proliferatif diyabetik retinopatiye bağlı gelişen vitreus hemorajisinin (VH) çekilmesinde intravitreal bevacizumab’ın (İVB) etkinliğini değerlendirmek.
Gereç ve Yöntem: Proliferatif diyabetik retinopati nedeni ile VH gelişen 70 hastanın (43 erkek, ortalama yaş 55.6±12.2 yıl) 70 gözü geriye dönük olarak değerlendirildi. Hastaların demografik özellikleri, başlangıç ve sonuç bulguları ile yapılan girişimler kaydedildi. İVB enjeksiyonu yapılarak takip edilen olgular (Grup 1, N=29) ile İVB yapılmadan takip edilen olgular (Grup 2, N=41) VH temizlenme zamanı ve cerrahiye gitme oranı açısından kıyaslandı.
Bulgular: Ortalama takip süresi Grup 1’de 14.5±6.1 ay ve Grup 2’de 18.4±9.6 ay idi (p=0.185). Ortalama görme keskinliği başlangıçta ve son muayenede iki grup arasında benzer bulundu (tümü için p>0.05). Grup 1’de % 86 olguya, Grup 2’de ise % 58 olguya ilk 1 ay içerisinde panretinal fotokoagülasyon uygulanabildi (p=0.016). VH temizlenme zamanı iki grupta farklı değildi (Grup 1’de 2.3±2.1 ay ve Grup 2’de 3.4±2.6 ay, p=0.146). Cerrahi gerektiren olgu sayısı Grup 1’de 7 (% 24.1) iken Grup 2’de 20 (%48.8 ) idi (p=0.048).
Sonuç: Proliferatif diyabetik retinopatiye bağlı VH gelişen olgularda intravitreal bevacizumab sonuç görme keskinliğini etkilemese de cerrahiye olan gereksinimin az olması ve erken dönemde panretinal fotokoagülasyon yapılabilen olgu sayısının daha fazla olması açısından etkili bulunmuştur.
