Olfactory neuroblastoma; A rare cause for external ophthalmoplegia, proptosis and compressive optic neuropathy

Olfactory neuroblastoma which is neuroectodermal tumor of the nasal cavity is a rare and locally aggressive malignancy that may invades the orbit via local destruction. In this study, a case with proptosis, external ophthalmoplegia and compressive optic neuropathy caused by ONB was reported. A detailed clinical examination, including ocular imaging and histopathological studies were performed. A 62-year-old female patient presented to our clinic with complaints of left eye proptosis and visual deterioration. Her complaints were started 2 months prior to admission. Visual acuity in the left eye was 2 meters finger count. There was relative afferent pupillary defect. She had 6 mm of proptosis, and limitation of motility. Fundus examination was normal in right eye, but there was a hyperemic disc, increased vascular tortuosity and dilation of the retinal veins in left eye. Computerized tomography scan and magnetic resonance imaging of the brain and orbits demonstrated a large heterogeneous mass in the left superior nasal cavity with extensions into ethmoidal sinuses, as well as into the left orbita compressing the medial rectus muscle and optic nerve. Endoscopic biopsy of the lesion was consistent with an olfactory neuroblastoma (Hyam’s grade 3). Orbital invasion may occur in patients with ONB. Therefore, it is important to be aware of this malignancy because some of patients will present with ophthalmic signs such as external ophthalmoplegia, proptosis, or compressive optic neuropathy.

Olfaktör nöroblastom; Eksternal oftalmopleji, proptozis ve kompresif optik nöropati için nadir bir neden

Nazal boşluğun nöroektodermal tümörü olan olfaktör nöroblastom (ONB), nadir görülen ve lokal olarak tahrip edici şekilde orbitayı invaze edebilen lokal agresif bir malignitedir. Bu çalışmada, ONB nedeniyle proptozis, eksternal oftalmopleji ve kompresif optik nöropatisi olan bir olgu sunuldu. Oküler görüntüleme ve histopatolojik çalışmalar da dahil olmak üzere ayrıntılı bir klinik muayene yapıldı. 62 yaşındaki kadın hasta kliniğimize sol gözde proptozis ve görme azalması yakınmaları ile başvurdu. Şikâyetleri başvurudan 2 ay önce başlamıştı. Sol gözdeki görme keskinliği 2 metreden parmak sayma idi. Rölatif afferent pupilla defekti mevcuttu. Hastada 6 mm'lik proptozis ve hareket kısıtlılığı vardı. Fundus muayenesi sağ gözde normal idi, ancak; sol gözde hiperemik disk, retina venlerinde artmış vasküler kıvrım ve genişleme vardı. Beyin ve orbitanın bilgisayarlı tomografi ve manyetik rezonans görüntülemesi, sol üst burun boşluğunda etmoidal sinüslerin yanısıra sol orbitaya doğru uzanan medial rektus kası ve optik siniri sıkıştıran geniş heterojen bir kitle gösterdi. Lezyonun endoskopik biyopsisi ONB ile uyumlu idi (Hyams evre 3). ONB'li hastalarda orbita invazyonu görülebilir. Bu nedenle, bu hastaların bazıları eksternal oftalmopleji, proptozis veya kompresif optik nöropati gibi oftalmik bulgularla karşımıza çıkacağından bu malignitenin farkında olmak önemlidir.
