Cyclic esotropia: Surgical treatment results of two cases

Cyclic esotropia is a rare form of strabismus characterized by alternating periods of esotropia and orthophoric (or almost orthophoric) eye position. We have discussed two children with cyclic esotropia and their surgical treatment results. A six year old boy was brought to our clinic with the typical complaint of a convergent squint which had been appearing and disappearing in a regular 24-hour cycle over a period of four years. He had a large deviation angle with associated suppression and no binocular function for a 24-hour cyclic pattern, but he had a small angle of esotropia with fusion and stereopsis for another 24 hours. He had undergone strabismus surgery on the left eye at another clinic 3 years ago. The other patient was a ten-year-old girl who was brought to our clinic with the reoccurring complaint of esotropia on alternating days over a period of four years. We determined that a large deviation angle with near- distance disparity and a 24-hour cyclic pattern were present.
For both cases, we planned strabismus surgery according to the amount of deviation on the esotropic day. Following the surgery, the eyes have continued to remain permanently straight during the follow-up period.

Siklik esotropya, esotropya ve ortoforik( hemen hemen ortoforik) göz pozisyonunun değişen peryotları ile karakterize şaşılığın nadir bir formudur. Biz siklik ezotropyalı iki çocuk ve onların cerrahi tedavi sonuçlarını tartışmaktayız. Altı yaşında bir erkek çocuk, tipik olarak 4 yıllık bir süre içinde, düzenli 24 saatlik bir siklüsle oluşan ve kaybolan içe şaşılık şikayeti ile kliniğimize getirildi. Hasta 24 saatlik bir siklüs için, supresyon veya binokuler fonksiyon yokluğu ile birlikte geniş bir kayma açısına sahipti, fakat diğer 24 saat için füzyon ve stereopsis ile birlikte küçük açılı bir esotropyaya sahipti. Hasta üç yıl önce diğer bir klinikte sol gözünden şaşılık cerrahisi geçirmişti. Diğer hasta 4 yıllık bir süre içinde, gün aşırı oluşan, içe şaşılık şikayeti ile kliniğimize getirilen on yaşında bir kız çocuğuydu. Biz, 24 saatlik bir siklüs için, yakın-uzak farklı, geniş bir kayma açısının mevcut olduğunu belirledik.
Biz her iki vaka için, şaşılık cerrahisini esotropik gündeki kaymanın miktarına göre planladık. Cerrahiyi takiben, gözler takip süresi boyunca kalıcı olarak paralel kalmaya devam etti.
