Comparing Conjunctival Changes ın Frequent Replacement Hydrogel and Frequent Replacement Silicone Hydrogel Contact Lens Users with Impression Cytology

PURPOSE: To compare the level of conjunctival changes between the frequent replacement hydrogel and frequent replacement silicone hydrogel contact lens users with conjunctival impression cytology.
MATERIAL-METHOD: Fourty-two contact lens users who were seen at the Cornea and Contact Lens Unit, Department of Ophthalmology, Dokuz Eylül University were evaluated in this study. The first group consisted of frequent replacement hydrogel (FRHL) contact lens users for minimum one year and maximum five years. The second group consisted of frequent replacement silicone hydrogel (FRSHL) contact lens users for minimum one year and maximum 5 years. Twenty healthy individuals with no contact lens history were included in the control group.
Conjunctival impression cytology was applied to all contact lens users and the control group in order to evaluate conjunctival changes and the results for impression cytology was graded by Nelson method.
RESULTS: In the comparison of groups according to impression cytology scoring, there was no difference between the users of frequent replacement hydogel lens and frequent replacement silicone hydrogel lens. Similar amounts of squamous metaplasia and goblet cell loss were encountered in both groups. Nonetheless, impression cytology grading was significantly lower in the control group than the other two groups.
CONCLUSION: In conclusion, the use of frequent replacement hydogel and frequent replacement silicone hydrogel lenses for over a year causes some histological changes in the conjunctiva such as squamous metaplasia and goblet cell loss eventually causing dry eye symptoms. However, no differences were determined between frequent replacement hydrogel and frequent replacement silicone hydrogel users with regard to the severity of conjunctival changes.
Sık Değişim Hidrojel ve Sık Değişim Silikon Hidrojel Kontakt Lens Kullanan Hastalarda Konjonktival Değişikliklerin İmpresyon Sitolojisi Yöntemi ile Karşılaştırılması

AMAÇ: Sık değişim hidrojel grubu ve sık değişim silikon hidrojel grubu kontakt lens kullanıcılarında konjonktival değişikliklerin impresyon sitolojisi yöntemi ile histolojik olarak karşılaştırılması.
MATERYAL-METOD: Dokuz Eylül Üniversitesi Tıp Fakültesi, Göz Hastalıkları Anabilim Dalı, Kornea ve Kontakt Lens Birimi’ne, kontakt lens kullanımı nedeni ile başvuran 42 hasta çalışma kapsamına alındı. Birinci grup en az 1, en fazla 5 yıldır sık değişim hidrojel (SDHL) grubu kontakt lens kullanıcılarından, ikinci grup en az 1, en fazla 5 yıldır sık değişim silikon hidrojel (SDSHL) grubu kontakt lens kullanıcılarından oluşturuldu. Kontrol grubuna 20 adet daha önce hiç kontakt lens kullanım öyküsü olmayan sağlıklı bireyler dahil edildi. Tüm kontakt lens kullanıcıları ve kontrol grubuna konjonktival değişiklikleri değerlendirmek amacı ile konjonktival impresyon sitolojisi uygulandı ve impresyon sitoloji sonuçları Nelson yöntemi ile derecelendirildi.
BULGULAR: İmpresyon sitolojisi puanı açısından gruplar arası karşılaştırmalarda; sık değişim hidrojel lens ve sık değişim silikon hidrojel lens kullanıcıları arasında bir farklılığın olmadığı, her iki grupta da skuamöz metaplazi ve goblet hücre kaybına benzer düzeyde rastlandığı, buna karşın impresyon sitolojisi derecesinin kontrol grubunda diğer iki gruba göre anlamlı olarak daha düşük olduğu saptandı.
SONUÇ: Sonuç olarak sık değişim hidrojel ve sık değişim silikon hidrojel lens kullanımı konjonktivada skuamöz metaplazi, goblet hücre kaybı gibi kuru göze neden olabilecek histolojik değişikliklere neden olmaktadır. Ancak konjonktival değişiklikler açısından her iki grup kontakt lens materyalleri arasında fark saptanmamıştır.
