Etiopathogenesis of Retinopathy of Prematurity

Increased number of newborn intensive care units and technological improvements lead to survival of increased number of immature infants which in turn leads to increased number of premature infants to be screened. Recently, understanding the specific effects of molecules such as insulin like growth factor -1 (IGF-1), erythropoietin (EPO), vascular endothelial growth factor (VEGF) that play a role in the etiopathogenesis of ROP and determining levels of these factors in the blood of premature infants and screening of infants with definitive blood levels of these factors are recommended. The aim of this review is to evaluate etiopathogenesis of ROP disease and determine the specific mediators that play a role in the pathogenesis of ROP disease. Understanding the pathogenesis of ROP may induce changes on screening, monitoring and treatment protocols.

Yenidoğan yoğun bakım ünitelerinin yaygınlaşması ve teknolojik gelişmelerle birlikte daha fazla sayıda ve daha gelişmemiş bebeklerin yaşatılması mümkün olmaktadır, bu da taranması gereken prematür bebek sayısını arttırmaktadır. Bu nedenle son zamanlarda PR’nin etyopatogenezinde rol oynayan insülin benzeri büyüme faktörü (IGF-1), eritropoetin (EPO), vasküler endotelyal büyüme faktörü (VEGF) gibi moleküllerin patogenezdeki rollerinin tam anlaşılması ve bu faktörlerin kan seviyelerinin tespit edilerek kan değerleri belirli seviyelerde olan bebeklerin taranması fikri gündemdedir. Bu derlemedeki amacımız güncel bilgiler ışığında PR’nin etyopatogenezini ve etyopatogenezde rol alan faktörleri tekrar değerlendirmektir. PR’nin etyopatogenezinin anlaşılması tarama, takip ve tedavi protokollerinin yakın zamanda yenilenmesini gündeme getirebilir.
