The superficial epithelium of the cornea, which is renewed continuously by corneal stem cells, plays a critical role for permanency of transparency. These stem cells are localized at the cornea-conjunctival transition zone, which is called “limbus”. When this zone is affected/destroyed, limbal stem cell deficiency (LSCD) ensues. Loss of limbal stem cell (LSC) function allows colonization of the corneal surface by conjunctival epithelium. Worldwide, over 6 million people are affected with corneal blindness, and LSCD is one of the most important reasons. Fortunately, it is becoming possible to recover vision by autologous transplantation of limbal cells obtained from the contra-lateral eye at least for the cases, in which only one eye is affected. Due to the potential risks that can affect the donor eye, only a small number of cells can be obtained, among which only 1-2% of the limbal epithelial cells are actually LSC. Vigorous attempts are being made to expand LSCs in culture for preserving or even enriching the stem cell population. Pellegrini et al (1997) first reported the ex vivo expanded LSCs as a treatment option in LSCD. Through these 19 years, various protocols for cultivation of limbal epithelial cells were experienced. It is still not clear which method promotes effective stem cell viability and a high-effort is being put on this area. The most preferred technique for limbal cell culture is the explant culture model. In this approach, a small donor eye limbal biopsy is placed as an explant in a biocompatible substratum for expansion. The outgrowth is then surgically transferred to the recipient eye. Korneanın saydam olabilmesi için ise diferansiyasyonunu tamamlamış yüzeyel hücrelerin dökülmesi ile sürekli yenilenen epitel tabakasının düzenli ve avasküler yapısını koruması zorunludur. Yenilenen epitel hücrelerinin kaynağı, limbus bölgesinde yerleşen kök hücrelerdir. Kornea vaskülarizasyonu ve opaklaşma ile giden körlük, tüm dünyadaki körlüklerin yaklaşık olarak %10’unu oluşturmaktadır (6-10 milyon kişi) ve limbal kök hücre yetmezliğinde bu türde körlük yapan faktörlerin başında gelmektedir. Kimyasal/ısı yanıkları, Steven Johnson sendromu, konjenital/ genetik anomaliler gibi birincil veya ikincil herhangi bir nedenle limbal kök hücrelerin fonksiyonunu kaybetmesi, konjonktiva epitelinin kornea üzerine yürümesine, skar-vaskülarizasyon gelişimiyle ciddi görme kaybı oluşmasına neden olmaktadır.
