The Tear Osmolarity Changes After Cataract Surgery.

Purpose: To determine the tear osmolarity changes of patients after phacoemulsification surgery.
Materials and Methods: Tear osmolarity measurements were performed at 30 eyes of 30 patients who had cataract surgery without any complication. Measurements were performed before surgery and consecutively at 1st month, 3rd month, and 6th month after the surgery. TearLab osmometer (TearLab Corporation, San Diego, CA, USA) device was used for the measurements and paired-t test was used for statistical analysis.
Results: The mean age of the patients was 72.3±3.7 (67-78) years. Thirteen patients were men 17 patients were women. The mean osmolarity values were 305.8±6.5 mOsm/L before the surgery and 312.3±6.4 mOsm/L at 1 st month, 307.5±5.1 mOsm/L at 3rd month and 305.1±5.7 at 6th month after the surgery. The difference between the values before the surgery and the 1st month was found statistically significant. (p<0.001)The difference between the values before the surgery and the 3rd and 6th month postoperatively was not significant. (p>0.001)
Conclusions: The tear osmolarity increases at the first month after surgery but decreases to the levels measured before surgery at the 3rd month. The increase at the first month maybe due to the corneal incisions and medication used after the surgery. We think that we have to take into account of this similarity increase in all cataract patients especially who also have dry eye disease.
Katarakt Cerrahisi Sonrası Gözyaşı Osmolaritesi Değişiklikleri

AMAÇ: Katarakt cerrahisi uygulanan olgularda gözyaşı osmolarite değişikliklerinin saptanması
GEREÇ VE YÖNTEM: Sorunsuz fakoemülsifikasyon cerrahisi uygulanan 30 olgunun 30 gözünde cerrahi öncesi, cerrahi sonrası 1.ay, 3.ay ve 6. ayda gözyaşı osmolarite ölçümü yapıldı. Ölçümler için TearLab osmometre cihazı (TearLab Corporation, San Diego, CA, USA ), istatistiksel değerlendirmede ise paired –t test kullanıldı.
BULGULAR: Olguların yaş ortalamaları 72.3±3.7 (67-78) yıldı. Onüç olgu erkek, 17 olgu kadındı. Olguların osmolarite ölçüm ortalama değerleri cerrahi öncesi 305.8±6.5 mOsm/L, cerrahi sonrası 1.ayda 312.3±6.4 mOsm/L, cerrahi sonrası 3.ayda 307.5±5.1 mOsm/L, cerrahi sonrası 6. ayda 305.1±5.7 mOsm/L olarak tespit edildi. Cerrahi öncesi ve cerrahi sonrası 1.ay osmolarite değerleri arasında istatistiksel olarak anlamlı bir fark bulundu (p<0.001) Cerrahi öncesi değerler ile cerrahi sonrası 3.ay ve 6. ay arasında anlamlı fark yoktu. (p>0.001)
SONUÇLAR: Cerrahi sonrası 1.ayda gözyaşı osmolarite değeri anlamlı olarak artmakta ancak 3. ayda cerrahi öncesi seviyelere dönmektedir. Birinci aydaki bu osmolarite artışı cerrahi kesilere ve cerrahi sonrası kullanılan ilaçlara bağlı olabilir. Tüm katarakt cerrahisi uygulanan olgularda ve özellikle kuru göz hastalarında bu bulguyu dikkate almak gerektiğini düşünmekteyiz.
