Purtscher-like retinopathy associated with atypical hemolytic uremic syndrome

A 25-year-old woman presented with acute bilateral blurred vision and history of headache, dizziness and syncope for three days. Her visual acuity was 20/60 in both eyes. Fundoscopy revealed bilateral peripapillary multiple yellow-white patches like cotton wool spots, intraretinal hemorrhages and macular edema. The patient was diagnosed as Purtscher-like retinopathy with the retinal findings and lack of trauma history. She was urgently hospitalized to the nephrology clinic with the thrombotic microangiopathy findings (hemoglobinemia, thrombocytopenia and acute renal failure). After the differential diagnosis of thrombotic microangiopathy; the patient was diagnosed as atypical hemolytic uremic syndrome (aHUS) with the clinical and laboratory findings. Eculizumab treatment was added to hemodialysis and plasmapheresis therapy. Three months after the treatment, retinal lesions recovered and visual acuity increased to 20/20 in her both eyes. To the best of our knowledge, this is the first reported case of Purtscher-like retinopathy associated with aHUS.

Atipik hemolitik üremik sendrom ilişkili Purtscher benzeri retinopati

25 yaşında kadın hasta üç gündür devam eden her iki gözde bulanık görme, baş ağrısı, baş dönmesi ve bayılma öyküsü ile kliniğimize başvurdu. Hastanın görme keskinliği her iki gözde 20/60 seviyesindeydi. Fundus muayenesinde her iki gözde optik disk çevresinde atılmış pamuk manzarasına benzeyen çok sayıda sarı-beyaz yama tarzi leyzonlar, retina içi kanama odakları ve makula ödemi görüldü. Hastaya travma öyküsü olmaması ve retina bulguları nedeniyle Purtscher benzeri retinopati tanısı konuldu. Hastanın eşlik eden trombotik mikroanjiopati bulguları (hemoglobinemi, trombositopeni ve akut böbrek yetmezliği) nedeniyle ivedilikle nefroloji kliniğine yatışı yapıldı. Trombotik mikroanjiopatinin ayırıcı tanısı yapılarak klinik ve laboratuvar bulguları ışığında hastaya atipik hemolitik üremik sendrom (aHÜS) tanısı konuldu. Mevcut hemodiyaliz ve plazmaferez tedavisine eculizumab tedavisi eklendi. Tedavi başlandıktan 3 ay sonra, hastanın retinal lezyonları geriledi ve görme keskinliği her iki gözde 20/20 seviyesine ulaştı. Bildiğimiz kadarıyla bu olgu, literatürde bildirilen ilk aHÜS’ e bağlı Purtscher benzeri retinopati olgusudur
