Late Disciform Endotheliitis after LASIK

Twenty-five year-old male was applied with a chief complaint of blurred vision in the left eye. In the history he had herpetic keratitits and was underwent LASIK procedure in 2005. Oral and topical acyclovir treatment was started one week ago when his complaints were started. His visual acuity was 0.2 and intraocular pressure was 14 mmHg on the left eye. Slit-lamp revealed deep central disciform edema, keratic presipitates and 1-2 positive cells in the anterior chamber. Although the patient had myopic LASIK ablation, the central corneal thickness was 652 microns thick. Scheimpflug imaging was drawing attention to the thickening of the posterior cornea and keratic precipitates. Sagital and anterior elevation maps were not effected, however posterior elevation map showed marked central flattening. Asphericity value Q was within normal limits in the anteriorly (Q=-0,14) and it was in oblate appearance posteriorly (Q=+5,24). In addition to the antiviral medications a topical dexamethasone treatment was started. Quick response was achieved within 4 days of treatment. The vision improved to 0.6 and edema was markedly reduced. Scheimpflug imaging parameters were significantly improved.

Faik Oruçoğlu, Ali AksuBirinci Göz Hastanesi, İstanbulLASIK sonrası geç dönem herpetik keratoüveit geçiren olgunun klinik ve görüntüleme özellikleri araştırıldı. Yirmi beş yaşında erkek hasta sol gözde bulanık görme ve fotofobi şikayeti ile baş vurdu. Hastanın hikayesinde daha önce aynı gözden herpes keratititi geçirdiği ve 2005 yılında LASIK olduğu öğrenildi. Bir hafta önce başlayan şikayetleri sebebi ile başvurduğu bir klinkte oral ve topikal asiklovir tedavisi başlanmıştır. Hastanın görmesi 0,2 düzeyinde idi. Göz içi basıncı 14 mmHg idi. Derin santral diskiform ödem mevcuttu. Yaygın keratik prespitatlar ve 1-2 pozitif ön kamara hücreleri vardı. Hastanın miyop LASIK olmasına rağmen santral kornea kalınlığı 652 mikron ölçüldü. Scheimpflug görüntülemede keratik prespitatlar ve posterior kalınlaşma dikkat çekmekte idi. Sagital ve ön elevasyon haritalarında belirgin değişiklik yoktu ancak arka elevasyonda santralin düzleştiği görüldü. Asferisite değeri olan Q değeri, ön korneada normal sınırlarda (-0,14), arka korneada ise yüksek oblate (+5,24) özellikte idi. Tedavide aldığı antivirallere ilaveten topikal deksametazon başlandı. Tedaviye 4. gün sonunda yanıt alındı. Görme 0,6 düzeylerine çıktı ve ödemde belirgin gerileme sağlandı. Scheimpflug görüntüleme parametrelerinde belirgin iyileşme oldu.
