The Factors Affecting the Incidence of Ptosis after Trabeculectomy

Methods: A total of312 patients (339 eyes) who underwent trabeculectomy with mitomycin-C between 2015 and 2020 were retrospectively evaluated. Patients who had a regular follow-up of at least 6 months and no history of ptosis or ptosis surgery were included. Age, sex, glaucoma types, preoperative and postoperative intraocular pressure(IOP), preoperative and postoperative anti-glaucoma drug, number of antiglaucoma drops, duration of anti-glaucoma drugs use, history of eye itching due to anti-glaucoma drug-associated allergy, duration of follow-up, post-operative needling, needling time, and ocular massage were recorded. Ptosis was defined as a reduction margin reflex distance1 (MRD1)≥2 mm from pre-operative levels. Ptosis that had not improved for at least 6 months was assesed as persistent ptosis. Multivariate logistic regression was used to determine potential predictors of ptosis development.
Results: Ptosis after trabeculectomy was observed in 35 of 339 eyes (10.3%). Thirty eyes of 30 patients (8.8%) had transient ptosis and 5 eyes of 4 patients (1.5%) had persistent ptosis. Preoperative duration of anti-glaucoma drug use, agent (prostaglandin analogs, beta-blockers, alpha2 agonists, and carbonic anhydrase inhibitors or combinations of these), needling time and ocular massage after trabeculectomy did not differ significantly between groups (p>0.05). Needling and eye itching due to anti-glaucoma drug-associated allergy were significantly higher in patients with ptosis (p<0.05).
Conclusion: Ptosis after trabeculectomy is an important problem for glaucoma patients.It has been observed that needling and eye itchingstory due to anti-glaucoma drug-associated allergy may increase the risk of ptosis.
Trabekülektomi Sonrası Pitozis İnsidansını Etkileyen Faktörler

Gereç ve Yöntem: 2015 ve 2020 yılları arasında mitomisin-C ile trabekülektomi cerrahisi yapılan 312 hasta (339 göz) retrospektif değerlendirildi. En az 6 ay düzenli takibi olan ve pitozis veya pitozis cerrahisi öyküsü olmayan hastalar dahil edildi. Yaş, cinsiyet, glokom tipleri,preoperatif ve postoperatif göz içi basıncı (GİB), preoperatif ve postoperatif anti-glokom ilaçları, anti-glokom damla sayısı, anti-glokom ilaç kullanım süresi, anti-glokom ilaç ilişkili alerjiye bağlı göz kaşıntı öyküsü, takip süresi, cerrahi sonrası bleb iğneleme, iğneleme zamanı, ve oküler masaj değerlendirildi. Pitozis, marjin refleks mesafesi 1 (MRD1), cerrahi öncesi değerden ≥2 mm azalması olarak tanımlandı. En az 6 ay düzelmeyen pitozis kalıcı pitozis olarak değerlendiriildi.Ptozis gelişiminin potansiyel tahmin edici faktörlerini bulmak için multivariate logistik regresyon analizi kullanıldı.
Bulgular: Trabekülektomi sonrası pitozis 339 gözün 35’inde izlendi (%10.3). 30 hastanın 30 gözünde (%8.8) geçici pitozis ve 4 hastanın 5 gözünde (%1.5) kalıcı pitozis izlendi. Preoperatif anti-glokom ilaç kullanım süresi, ilaç etken maddesi (prostaglandin analoğu, beta-blokörler, alfa 2 agonistler, ve karbonik anhidraz inhibitörleri veya bunların kombinasyonları), iğneleme zamanı ve trabekülektomi sonrası oküler masaj gruplar arasında anlamlı farklı değildi (p>0.05). İğneleme ve anti-glokom ilaç ilişkili alerjiye bağlı göz kaşıntısı pitozis olan hastalarda istatistiksel anlamlı olarak daha yüksekti (p<0.05).
Sonuç: Trabekülektomi sonrası görülen pitozis glokom hastaları içinönemli bir problemdir. İğneleme ve anti-glokom ilaçlarla ilişkili alerjiye bağlı göz kaşıma hikayesinin pitozis riskini arttırabileceği gözlenmiştir.
