Comparison of the effect of trabeculectomy with mitomycin C on macular thickness in primary open-angle glaucoma and pseudoexfoliative glaucoma

Patients and methods: 13 eyes with PXG and 11 eyes with POAG, which had undergone trabeculectomy with mit C, were included to our prospective study. Cases with histories of any systemic vascular diseases and previous ocular surgeries weren’t included. The CMT was measured before the surgery and at the first postoperative week and month by spectral-domain optical coherence tomography (SD-OCT). The values before and after the surgery of both of the groups and the differences between the two groups were compared statistically by Mann-Whitney U ve Wilcoxon signed rank tests.
Results: The means of CMT at the first postoperative week and month were statistically significantly higher than the preoperative values in both POAG and PXG groups. (POAG: 1st week p=0.0090, 1st month p=0.0086, PXG: 1st week p=0.0023, 1st month p=0.0050). But no statistically significant differences were found between the changes of preoperative and postoperative mean CMT values between the two groups (1st week p=0.4326, 1st month p=0.5028).
Conclusion: In spite of the increases in CMT at the 1st week and month of trabeculectomy with mit C in POAG and PXG, no significant differences were detected between the changes of CMT between the two groups. Based on the fact that more inflammation and higher decrease in intraocular pressure might be seen in PXG than POAG postoperatively, further investigations with great number of cases are planned.
Primer açık açılı glokom ve psödoeksfoliatif glokomda, mitomisin C’li trabekülektominin maküler kalınlık üzerine olan etkisinin kıyaslanması

Gereç ve yöntem: Mit C’li trabekülektomi cerrahisi planlanan PEG’lu 13 hastanın 13 gözü ve PAAG’lu 11 hastanın 11 gözü prospektif çalışmamıza dahil edildi. Sistemik vasküler hastalık ve geçirilmiş oküler cerrahi öyküsü olan olgular çalışma kapsamına alınmadı. MMK ölçümleri, cerrahi öncesinde ve cerrahi sonrası 1. hafta ve 1. ayda Spektral-Domain optik koherens tomografi (SD-OCT) kullanarak yapıldı. Cerrahi öncesi ve sonrası değerler, ayrıca gruplar arası farklılıklar Mann-Whitney U ve Wilcoxon-signed-rank testi ile istatistiksel olarak analiz edildi.
Bulgular: Gerek PAAG gerekse PEG’lu olgularda, cerrahi sonrası 1. hafta ve 1. aydaki ortalama MMK değerleri, cerrahi öncesi değerlere oranla, istatistiksel olarak anlamlı ölçüde yüksek bulundu (PAAG: 1. hafta p=0.0090, 1. ay p=0.0086; PEG: 1. hafta p=0.0023, 1. ay p=0.0050). Ancak her iki grup arasında, cerrahi sonrası 1. hafta ve 1. ay değerlerindeki artışlar arasında anlamlı fark gözlenmedi (1. hafta p=0.4326, 1. ay p=0.5028).
Tartışma: PAAG ve PEG’lu olgularda, cerrahi sonrası 1. hafta ve 1. aydaki ortalama MMK değerlerinde, cerrahi öncesi değerlere oranla artış görülse de, bu artışlarda her iki grup arasında anlamlı farka rastlanmadı. PEG’lu olgularda, cerrahi sonrası daha fazla inflamasyon ve göz içi basıncında daha fazla düşme ihtimali sebebiyle, MMK da daha fazla artış beklenmesiyle yola çıkılan bu çalışmanın, daha yüksek sayıda olgularla yapılması planlandı.
