Management of contact lenses and visual development in pediatric aphakia

Congenital cataract is one of the important causes of treatable vision loss in childhood. The first weeks and months of life are a critical time for the development of vision, and therefore the treatment of aphakia in the early stages of life is of great value for the management of vision development. Among the treatment models, contact lenses (CL) have an important place in infancy and early childhood up to the age of 2 years. While a good visual gain, especially in unilateral aphakia was not considered very likely, important steps have been taken in the treatment of pediatric aphakia thanks to the surgical techniques developed over time and the increasing experience in optical correction systems, especially in CL. In this review; with current developments, the types of CL used in pediatric aphakia, their application features, comparison with other optical systems, the features of amblyopia treatment in the presence of CL, the results obtained with the compliance of families to CL application and closure treatment are examined and shared in the light of existing studies. The limitations of this review are that the majority of the studies are retrospective, case series, the scarcity of prospective, multicenter studies, and the high variability of the data.

Konjenital katarakt çocukluk döneminin tedavi edilebilir görme kayıplarının önemli nedenlerinden biridir. Yaşamın ilk haftaları ve ayları görme duyusunun gelişimi için kritik bir zaman dilimidir. Bu nedenle erken katarakt cerrahisi ve sonrasında oluşan afakinin çok yönlü etkin tedavisi görme gelişiminin yönetimi açısından büyük değer kazanır. Tedavi modelleri arasında kontakt lensler (KL), infant ve 2 yaşa kadar olan erken çocukluk döneminde önemli bir yere sahiptir. Daha önceleri özellikle tek taraflı afaki konusunda iyi bir görme kazanımı çok olası görülmezken zamanla gelişen cerrahi yöntemler ve KL başta olmak üzere optik düzeltme sistemlerinde artan deneyim sayesinde pediatrik afakinin tedavisinde önemli adımlar atılmıştır. Bu derlemede; güncel gelişmelerle pediatrik afakide kullanılan KL tipleri, uygulama özellikleri, diğer optik sistemler ile karşılaştırılması, KL varlığında ambliyopi tedavisinin özellikleri, ailelerin KL uygulaması ve kapama tedavisine uyumu ile alınan sonuçlar mevcut çalışmaların ışığında irdelenerek paylaşılmıştır. Araştırmaların büyük çoğunluğunun geriye dönük, olgu serileri şeklinde olması, verilerin çok fazla çeşitlilik göstermesi ve ileriye yönelik, çok merkezli çalışmaların azlığı derlemenin sınırlamalarını oluşturmuştur.
