Staphylococcus Epidermidis Endophthalmitis Masquerading as Panuveitis after an Imperceptible Ocular Trauma

Prompt recognition and treatment are paramount to avoid irreversible visual loss. We present a case of severe panuveitis following ocular trauma with a tree branch that did not cause any evident ocular wound and discuss the difficulties in achieving a diagnosis that can allow proper treatment.
A 21-year-old healthy man presented with acute anterior uveitis. He was managed elsewhere with oral acyclovir and topical steroids for presumed herpetic uveitis. He subsequently developed severe panuveitis with profound decrease in vision. Diagnostic vitrectomy was performed and vitreous samples were positive for Staphylococcus epidermidis. Systemic and intravitreal antibiotic therapy was initiated and after five days, the patient recovered with a remarkable improvement in visual acuity to 6/12.
Post-traumatic endophthalmitis can occur with an imperceptible trauma without an obvious compromise of the eye globe.

Acil tanı ve tedavi, geri dönüşümsüz görme kaybını önlemek için çok önemlidir. Ağaç dalı ile oluşan ve hiçbir oküler yaralanmaya sebep olmayan oküler travma sonrası gelişen şiddetli bir panüveit olgusu sunulmakta ve tanı ve tedavideki zorluklar tartışılmaktadır.
21 yaşında sağlıklı bir erkek, akut ön üveit tablosu ile başvurdu. Başka bir merkezde, herpetik üveit kabul edilerek oral asiklovir ve topikal steroid tedavisi verilmişti. Sonrasında hastada, şiddetli panüveit ve görme kaybı gelişti. Tanı için diagnostik vitrektomi uygulandı ve alınan vitreus örneğinde Staphylococcus epidermidis pozitif bulundu. Sistemik ve intravitreal antibiyotik tedavisi başlandıktan 5 gün sonra hastanın görme keskinliği 6/12’ye yükseldi.
Travma sonrası endoftalmi, fark edilmeyen bir travma sonrasında ve göz küresinde herhangi bir bulgu olmadan meydana gelebilir.
