Uveal Melanoma; Current Trends in The Diagnosis and ManagementBerçin Tarlan1, Hayyam Kıratlı21Private Practice, Ophthalmology, Ankara, Turkey2Department Of Ophthalmology, Hacettepe University, School Of Medicine, Ankara, Turkey Uveal melanoma which is the most common primary intraocular malignancy in adults arises from melanocytes within the iris, ciliary body and the choroid. The diagnosis is based principally on clinical examination of the tumor with the biomicroscopy and indirect ophthalmoscopy and confirmed by the diagnostic techniques such as ultrasonography, fundus fluorescein anjiography and optical coherence tomograpy. The clinical diagnosis of the posterior uveal melanomas can be made when the classical appearance of a pigmented dome shaped mass is detected on the dilated fundus exam. Uveal melanomas display classically low to medium reflectivity in A-scan ultrasonography and in a B-scan the tumor appears hyperechoic and acoustically hollow. Management of a small pigmented lesion is determined by its risk factors of transforming into a choroidal melanoma such as documentation of growth, thickness greater than 2 mm, presence of subretinal fluid, symptoms and orange pigment, margin within 3 mm of the optic disc, absence of halo and drusen. Advances in the diagnosis, local and systemic treatment of uveal melanoma have been achieved in the past few decades and caused a shifting from enucleation to eye-conserving treatment modalities including transpupillary thermotherapy and radiotherapy. Prognosis can be most accurately predicted by genetic profiling of fine needle aspiration biopsy of the tumor before the treatment and high-risk patients can now be identified for clinical trials that may lead to target-based therapies for metastatic disease and adjuvant therapy which aims to prevent metastatic disease. Tanıda biyomikroskopi ve indirekt oftalmoskopi ile yapılan klinik muayene esas alınır, ultrasonografi, fundus flöresein anjiyografi ve optik koherens tomografi gibi tanısal teknikler ile doğrulanır. Arka uvea melanomu tanısı, dilatasyonlu fundus muayenesinde klasik olarak pigmente, kubbe şeklinde kitlenin tespit edilmesiyle konur. Tümör A-mod ultrasonografide düşük-orta yansıma özelliği gösterirken, B-modda hiperekoik ve akustik boşluk görünümü veren kitle şeklinde görülür. Koroideada belirsiz pigmente küçük bir lezyonun yönetimi, lezyonda büyümenin gösterilmesi, kalınlığın 2 mm’den büyük olması, semptom, subretinal sıvı ve turuncu pigment mevcudiyeti, optik diske 3 mm’den daha yakın mesafede bulunması, halo ve druzen bulunmamasi gibi koroidea melanomuna dönüşüm lehine risk faktörleri göz önüne alınarak belirlenir. Son yıllarda meydana gelen tanı ve tedavideki gelişmeler tedavide enükleasyondan, transpupiller termoterapi, radyoterapi gibi göz koruyucu tedavi seçeneklerine kaymaya sebep olmuştur. Tedavi öncesinde tümörden ince iğne aspirasyon biyopsisinin genetik değerlendirilmesiyle prognoz öngörülebilir ve metastatik hastalığa yönelik tedaviyi ve önlemeyi hedef alan klinik çalışmalar için yüksek riskli hastalar belirlenebilir.
