Microbial Keratitis After Treatment of Collagen Crosslinking
Banu Torun Acar, Ece Turan Vural, Elvin Halili, Suphi AcarHaydarpasa Numune Education And Research Hospital, 2nd Ophthalmology ClinicA 33-year-old woman presented with a pain, redness, and diminution of vision occurring 2 days after collagen crosslinking had been performed for keratoconus in the right eye. Culture results from the patient's contact lens and corneal scrapings were positive for Stafilokokus epidermidis. According to the results of antibiotic susceptibility testing, the patient was treated with hourly topical fortified vancomycin and exocin. Before collagen crosslinking, the best corrected visual acuity (BCVA) was 4/10, the manifest refraction was -7.00 -1.755 3°. Four months after the procedure, the BCVA was 4/10, the manifest refraction was -5.50 -1.75 10°. Slitlamp examination revealed a mild residual haze in the upper midperipheral cornea, and stromal opacities had disappeared.
Collagen crosslinking is a minimally invasive method than other methods for treatment of keratoconus, but epithelial debridement and bandage contact lens wearing may lead to the development of bacterial keratitis.
Kollajen Çapraz Bağlama Tedavisi Sonrası Mikrobiyal Keratit

Göz KliniğiKeratokonus tanısı ile sağ gözüne kollajen çapraz bağlama tedavisi uygulanan 33 yaşındaki kadın hasta tedavi sonrası 2. günde sağ gözünde ağrı, kızarıklık, fotofobi, bulanık görme şikayeti ile başvurdu. Korneadan alınan sürüntü materyalinde ve bandaj kontakt lenste Stafilokok epidermidis üredi. Antibiyotik duyarlılık testi sonuçlarına göre hasta saat başı topikal güçlendirilmiş vankomisin ve %0.5 moksifloksasin ile tedavi edildi. Çapraz bağlama tedavisi öncesi hastanın en iyi düzeltilmiş görme keskinliği 4/10, manifest refraksiyonu -7.00 -1.75 3° idi. Çapraz bağlama tedavisinden 4 ay sonra en iyi düzeltilmiş görme keskinliği 4/10, manifest refraksiyonu -5.50 -1.75 10° olarak değerlendirildi. Biyomikroskopik incelemede kornea midperiferinde minimal haze kaldığı, stromal opasitelerin kaybolduğu görüldü.
Kollajen çapraz bağlama tedavisi keratokonus tedavisinde diğer yöntemlere göre daha az invaziv olsa da epitel debridmanı yapılması ve bandaj kontakt lens kullanımı bakteriyel keratit gelişimine neden olabilmektedir.
