6 Months Results of DMEK for 100 Eyes: First Clinical Results from Turkey

Methods: Medical records of the patients for whom DMEK surgery was performed were reviewed retrospectively. Best corrected visual acuity (BCVA), donor corneal endothelial cell density (ECD), donor age, duration in solution after obtaining the donor tissue, and duration after exitus of cadaver were evaluated preoperatively and BCVA, ECD, and ECD loss (%) at the postoperative 6th month were evaluated postoperatively. Graft detachment, graft failure, and pupillary block were recorded as surgical complications. For the patients who had cataract and FED, combined or staged procedures were performed. Two different donor preparation techniques were utilized; 8 and 9.5 mm.
Results: A hundred eyes of the 74 patients were included in the study. 52 of the eyes had FED and the remaining 48 had PBK. The mean ECD loss in 6 months was 29.2±4.4 % in the FED group and 29.7±5 % in the PBK group (p=0.415). The mean BCVA was 0.06±0.05 at 6 months in the patients with FED whereas it was 0.07±0.05 at 6 months in the patients with PBK (p=0.378). The mean ECD loss in 6 months was 28.3±5.3 % in the 8 mm group and 29.7±4.5 % in the 9.5 mm group (p=0.255). The mean ECD loss in 6 months was 28.5±5.6 % in the combined group and 29.8±2.9 % in the staged group (p=0.279).
Conclusion: Different donor preparation techniques can be utilized with similar efficiency for DMEK surgery. A staged or combined approach can be used efficiently in the management of the patients with FED and cataract. Our results are promising both for the PBK and FED patients.

Materyal ve Metod: DMEK cerrahisi yapılan hastaların tıbbi kayıtları retrospektif olarak değerlendirilmiştir. Preoperatif olarak en iyi düzeltilmiş görme keskinliği (EİDGK), donör korneal endotelyal hücre yoğunluğu (EHY), donör yaşı, donör dokunun alınmasından sonra solüsyonda bekleme süresi ve kadavranın ölümünden donör dokunun alınmasına kadar geçen süre; postoperatif 6. ayda ise EİDGK, EHY ve EHY kaybı (%) değerlendirmeye alınmıştır. Greft dekolmanı, greft yetmezliği ve pupiller blok cerrahi komplikasyonlar olarak ele alınmıştır. Katarakt ve FED olan hastalarda kombine veya aşamalı cerrahi uygulanmıştır. 2 farklı donör hazırlama tekniği kullanılmıştır: 8 ve 9,5 mm.
Bulgular: 74 hastanın 100 gözü çalışmaya dahil edilmiştir. 52 gözde FED, 48 gözde ise PBK mevcuttu. FED hastalarındaki 6. ayda EHY kaybı %29,2±4,4 iken, PBK hastalarında %29,7±5 olarak bulunmuştur (p=0,415). 6.aydaki ortalama EİDGK FED hastalarında 0,06±0,05 iken, PBK hastalarında 0,07±0,05 olarak bulunmuştur (p=0.378). 8 mm donör tekniği kullanılan grupta 6. aydaki EHY kaybı %28,3±5,3 iken, 9,5 mm donör tekniği kullanılan grupta %29,7±4,5 olarak bulunmuştur (p=0,255). Kombine cerrahi uygulanan grupta 6.aydaki EHY kaybı %28,5±5,6 iken, aşamalı cerrahi grubunda %29,8±2,9 olarak bulunmuştur (p=0.279).
Sonuç: DMEK cerrahisinde farklı donör hazırlama teknikleri tercihi aynı etkinlikte bulunmuştur. FED ve kataraktı olan hastalarda aşamalı veya kombine yaklaşım benzer etkinlikte uygulanabilir. Sonuçlarımız hem FED hem de PBK hastaları için umut vericidir.
