Rituximab treatment in a patients with active Graves' orbitopathy and psoriasis

Management of Graves’ orbithopathy remains an important therapeutic challange. Current therapeutic modalities are unsatisfactory in about one third of the patients. Rituximab is a monoclonal antibody against CD 20 antigen that is expressed in mature and immature B cells. Eearly experience with rituximab suggests that it is a promising alternative therapy for Graves’ orbithopathy. Here we report a case of a 49 years old woman with Graves’ orbitopathy and psoriasis. The patient received two infusions of 1 g rituximab 2 weeks apart. Although, there was improvement in inflammatory signs of the disease, proptosis did not change after the treatment.

Graves orbitopatinin tedavisi halen önemli bir sorun teşkil etmektedir. Günümüzde uygulanmakta olan tedavi yaklaşımları hastaların üçte birinde etkili olmamaktadır. Rituksimab olgunlaşmış ve olgunlaşmamış B hücrelerinde bulunan CD 20 antijenine karşı geliştirilmiş monoklonal antikor olup Graves orbitopati tedavisinde alternatif bir yöntem olarak ümit vaadetmektedir. Bu makalede Graves orbitopati ile beraber psöriazis olan 49 yaşında bir olgu sunulmuştur. Hastaya 2 hafta arayla 1g rituksimab ibtravenöz olarak verildi. Tedavi sonrası hastalığa ait inflamatuar bulgular gerilemekle birlikte propitozisde değişiklik olmadı.
