Occult Macular Dystrophy

In this case report, a 20 year-old female patient with bilateral progressive decline of visual acuity for six years was reported. Her visual acuity was 3-4/10 in both eyes. Anterior segment and fundus examination, fluorescein angiograms and full field electroretinograms were normal. She could read all Ishara pseudoisochromatic plates. Fundus autofluorescence imaging was normal. There was a central mild hyporeflectance by fundus infrared reflectance imaging in both eyes. Decrease in foveal thickness and alterations of the photoreceptor inner and outer segment junction was observed by optical cohorens tomograghy in both eyes. Central scotoma was also found by microperimetry and reduced central response was revealed by multifocal electroretinogram in both eyes. These findings are consistent with the clinical characteristics of occult macular dystrophy.

Işıl Sayman Muslubaş, Serra Arf, Mümin Hocaoğlu, Hakan Özdemir, Murat Karaçorluİstanbul Retina Enstitüsü, İstanbul,türkiyeOkült makuler distofi, her iki gözde ilerleyici görme kaybı ile seyreden, fundus görünümü, floresein anjiografi incelemesi ve tüm alan elektroretinogram tetkiki normal olan, kalıtımsal bir makuler distrofidir. Bu olgu sunumunda 6 yıldır her iki gözde ilerleyici görme kaybı olan 20 yaşında kadın hasta sunulmaktadır. Görme keskinliği her iki gözde 3-4/10 düzeyinde idi. Ön segment ve fundus muayenesi, floresein anjiografi ve tüm alan elektroretinogram incelemesi normaldi. Ishihara renk görme testinde yakından bakarak tüm plakaları okudu. Fundus otofloresans incelemesi normaldi. Fundus infrared reflektans görüntüleme tekniğinde her iki gözde santral hafif hiporeflektans mevcuttu. Optik koherens incelemesinde her iki gözde fovea kalınlığında azalma ve fotoreseptör iç ve dış segment birleşiminde bozulma gözlendi. Mikroperimetri tetkikinde santral skotom ve multifokal elektroretinogram testinde ise santral cevabın azaldığı tespit edildi. Bu bulgular okult makuler distrofinin klinik özellikleriyle uyumlu olarak bulunmuştur.
